Job overview

This post is fixed term/secondment for 24 months due to meet the needs of the service

The successful candidate will join the Community Hospital Service based at Brecon Hospital, working within a supportive multidisciplinary team providing care for patients with frailty, rehabilitation and complex medical needs. The post offers an excellent opportunity to develop skills in community-based medicine, integrated care and the management of patients transitioning from acute hospital settings.

The post holder will work under the supervision of the Consultant Physician/ Specialist and will be supported to develop clinical, leadership and quality improvement skills. In keeping with Powys Teaching Health Board's integrated approach to care, there may also be opportunities to gain experience within Mental Health and Learning Disability services, depending on individual interests and service requirements, providing exposure to a broader range of community and integrated care pathways.

 

Main duties of the job

Qualifications

Essential criteria
  • MBBS or equivalent medical qualification
Desirable criteria
  • Relevant higher specialist qualification e.g. MRCGP, MRCP

Clinical training and experience

Essential criteria
  • • Shall have full GMC registration and a License to Practice.
  • • Significant years of post-registration clinical experience.
  • • Significant years experience of working in, General Internal Medicine, Geriatrics or other medical Specialty. Experience of diagnosis and management of common medical conditions in adults.
  • • Evidence of Continuing Professional Development Experience of the multidisciplinary approach in patient management.
  • • Experience in general medicine at the level of completion of Basic Core Training eg IMT
  • • Understanding of principles of medical audit.
Desirable criteria
  • Experience in Community Medicine. Experience in Rehabilitation Medicine, Neurology, Stroke Medicine

Fitness To Practise

  • Applicant’s knowledge is up to date and fit to practise safely.

  • • Good written and verbal communication skills Time management skills and ability to prioritise.
  • • Information technology skills and use of electronic record keeping.
