You will be required to work as part of a multidisciplinary team and to provide specialist assessment, treatment and care to a caseload of people with severe and enduring mental health difficulties from within the local community.

The successful candidate will be required to provide care to service users within the legal frameworks of the Mental Health (Wales) Measure 2010, the Social Services and Well- Being (Wales) Act 2014 and the Mental Health Act (1983)

You must be enthusiastic, self-motivated, flexible in approach, able to work autonomously as well as with statutory and non- statutory service providers. 

Effective communication and engagement skills are fundamental to this role, as well as a current evidence-based knowledge and a commitment to working with individuals adopting a recovery approach and a commitment to working within a trauma informed model of care.

Main duties of the job

Assist the Team Manager with the management and organization of their clinical area 
Participate in the single point of access meetings (SPOA) . Responsible for undertaking and documenting the Mental Health Measure paperwork, evidence-based assessments of client’s physical, psychological, spiritual and social needs, taking-into-account and respecting ethnicity, for a defined caseload. Assess and manage the care of clients with complex risk behaviours ensuring that a comprehensive risk assessment is completed and documented, for all actual or potential areas of risk, i.e. harm to self or others, neglect, exploitation and 
vulnerability. 
Responsibility for formulating and developing standard and complex, outcome focused care and treatment plans in 
collaboration with clients and in accordance with the multi-disciplinary care programme approach.  
Monitor and evaluate commissioned services utilized within the care packages.  Responsible for the care co-ordination of reviews in accordance with legislation incorporating statutory and legal 
requirements under the Mental Health Act 1983 and the Mental Capacity Act, ensuring dissemination of documentation as 
appropriate. 
Provide written and verbal reports for Mental Health Review Tribunals/Appeals and for other service requirements as necessary. 
Responsibility for the safe administration of prescribed depot medications, physical health monitoring in collaboration with GP and providing education (if appropriate to profession).
