Byddwch yn rhan o 'Bowys Iach a Gofalgar' a’n helpu ni i drawsnewid y ffordd rydym yn darparu gwasanaeth iechyd a gofal, fel y gall ein cymunedau Ddechrau'n Dda, Byw'n Dda a Heneiddio'n Dda.
Trosolwg o'r swydd
Swydd Cyfnod Penodol am 24 mis tan Chwefror 2028 oherwydd gofynion y gwasanaeth.
Bydd deiliad y swydd yn chwarae rhan allweddol wrth gefnogi datblygiad a gweithrediad y Rhaglen Drawsnewid yn unol â chanlyniadau strategol allweddol y Bwrdd Iechyd a chyflawni ei Gynllun Blynyddol. Mae'r rôl yn cefnogi'r gwasanaeth wrth yrru trawsnewid a gwerth; wrth gynllunio a chyflawni canlyniadau, profiad a chost gwell o fewn ailgynllunio'r system gyfan.
Bydd y Rheolwr Trawsnewid Gwasanaeth yn darparu ystod o weithgareddau ac arbenigedd i gefnogi'r Rhaglen Drawsnewid. Bydd hyn yn cynnwys gweithio'n annibynnol a chyda chydweithwyr, o fewn a thu allan i'r Bwrdd Iechyd, i ddylunio a datblygu cynnwys y Rhaglen ac i sicrhau llywodraethu priodol ar gyfer ailgynllunio llwybr y system gyfan.
Prif ddyletswyddau'r swydd
Datblygu'r Rhaglen i ddarparu'r gwasanaethau wedi'u hailgynllunio drwy drawsnewid, gwerth a llywodraethu rhaglenni priodol, gan gynnwys darparu amrywiaeth o brosiectau a ffrydiau gwaith.
Adolygu a dadansoddi gwasanaethau (gan gynnwys dadansoddi bylchau), gan wneud penderfyniadau ynghylch y budd posibl i gyflawni amcanion y Rhaglen.
Defnyddio methodoleg drawsnewid briodol, gan gynnwys er enghraifft, 'darganfod, dylunio, darparu', neu 'dadansoddi, cynllunio, gwneud, adolygu', i gynllunio a chefnogi datblygu a darparu rhaglenni fesul cam.
Rheoli effeithiol ystod o brosiectau a ffrydiau gwaith a fydd yn trawsnewid y ddarpariaeth gwasanaeth bresennol trwy nodi a chymhwyso'r sail dystiolaeth, cydymffurfio â pholisi cenedlaethol i drawsnewid polisïau, prosesau, strwythurau, ffyrdd o weithio a diwylliant.
Sicrhau llywodraethu rhaglenni cadarn gan gynnwys cynllun rhaglen, cofrestr risg, mapio rhanddeiliaid, adrodd uchafbwyntiau, cofnodion cywir, i gyflawni'r newid gofynnol.
Cydnabod problemau ansawdd a diogelwch, a pherfformiad a chanlyniadau o fewn y gwahanol wasanaethau a nodi meysydd i'w gwella.
Gweithio'n agos gyda chydweithwyr i sicrhau bod prosesau'n cydymffurfio â'r gofynion hanfodol, er enghraifft, nodi unrhyw angen am ymgysylltu ac ymgynghori.
Darparu ystod o fewnbynnau a gweithdai datblygu o fewn y rhaglen yn seiliedig ar wybodaeth meincnodi arfer gorau mewnol ac allanol mewn partneriaeth â rhanddeiliaid clinigol a rhanddeiliaid eraill.

Qualification and/or Knowledge

- Master’s Degree level qualification or demonstrable equivalent level of work experience
- Knowledge and experience of capacity/demand management and planning
- Evidence of continuous professional development by means of formal courses and/or workshops, study days etc
- IQT Bronze Award
- Knowledge of public sector contracting / procurement / commissioning processes
- Knowledge and experience in service redesign, service improvement, programme and or project management

- IQT Silver Award
- Project Management qualification such as PRINCE 2 or equivalent
Experience

- Management experience, preferably within an NHS environment, including management of projects / programmes within quality, time and financial parameters
- Experience of operating in a politically sensitive environment
- Experience of drafting briefing papers and correspondence at senior management team level
- Experience of undertaking service audits and analysing results against the relevant governance and Quality and Safety arrangements
- Experience of identifying and reporting risks
- Experience of designing and implementing effective transformation and/or service improvement programmes
- Effective team working to achieve objectives within integrated services model
- Appreciation of the relationship between risk management, audit and service improvement and experience of navigating this relationship to improve a service
- Demonstrate capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly
- Successful management of significant organisational change in a complex environment

- Experience of managing and motivating staff and reviewing performance of the individuals
- Experience of identifying and interpreting national policy
- Experience of researching best practice (global, private, and), interpreting its relevance and processes / practices which could be implemented successfully to achieve service transformation (advising on policy implementation)

- Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement

- Must have an understanding of the public sector in Wales and understand the landscape for integration and collaboration between organisations including crossborder
