Special School and Respite Team Lead

An exciting opportunity has arisen for an enthusiastic, forward thinking Community Children’s Nurses to join our leadership team as our Special School and Respite Team Lead.
We provide a special school nursing service to two special schools across Shropshire, Telford and Wrekin. We also have a small respite team of health care assistants providing packages of care to children and young people with a nursing need on our caseload.
This post will be an attractive position to a candidate looking to move into a leadership role and wishing to remain within the field of special school nursing and complex care.
We are looking for an experienced, children's nurse whose vision is to provide a quality service and to maximise a positive patient experience in this established service. The candidate must demonstrate extensive experience working with children and young people who has complex health and learning disabilities in the community setting.
The successful candidate will be responsible for the day to day leadership and management of the team, working closely with the CCN Team Lead and the Community Children's Nursing Service Manager,

- To effectively manage and lead the delivery and evaluation of the community children’s nurse-led initiatives including the Special School Nursing and Respite services within Shropshire, Telford and Wrekin.
- To work alongside multi-disciplinary, multi-agency teams across primary and secondary care, education and social care to deliver a needs led service for children and young people with chronic/complex/palliative/respite health care needs.
- To act as the CCN Service SEND lead.
The team leader will work autonomously in the community to ensure the service protects and improve the health and wellbeing of children with a range of health needs in achieving their full health and educational potential.
The team leader at times will provide telephone triage, patient consultations at school, home and within nurse-led clinic settings, undertake assessment, diagnosis and treatment and where appropriate nurse prescribing for children and young people with acute/chronic health care needs within agreed protocols.
The successful candidate will be required to work as part of a roster between Monday-Friday working flexibly between 08.00-18.00. The post holder will also be required to participate and provide leadership for the on-call provision to support end of life care over a 24 hour period.

Training and Qualifications

- NMC Registered Children's Nurse/RN (Child) or Registered School Nurse with SCPHN qualification
- Specialist Community Children’s Nursing qualification
- Evidence of short courses at master’s level or equivalent experience
- Student Nursing supervisor and assessor training
- Leadership and Management qualification
- Independent Nurse Prescriber (to be completed within 12months of commencement of post)
Experience

- Extensive post registration experience working with children and young people with nursing needs in the community setting.
- Experience of teaching and assessing in a clinical area.
- Participation in service development and implementation of change.
- Recent experience of providing clinical leadership and experience of line management.
- Experience of multi-disciplinary / multi-agency working.
Knowledge and Skills

- Evidence of continuing post registration education/training in field of practice
- Knowledge of relevant guidelines & ability to interpret their impact on strategic service developments, patient outcomes and clinical practice
- Ability to communicate complex & distressing information to CYP & their parent/carers in a sensitive & supportive way at a level they can understand
- Skilled on the use of IT
- Advanced communication, negotiation, decision making and organisational skills, both verbally and in writing
