Job overview

We are looking to recruit a Critical Care Outreach Practitioner (CCOP) to join our existing Critical Care Outreach Team (CCOT) at The Shrewsbury and Telford Hospitals NHS Trust - we have full time and part time positions available.

This is an exciting time to join the team as we redevelop and relaunch the service and work towards establishing and embedding a 24/7 Critical Care Outreach service Trust wide. We are looking for a compassionate, enthusiastic, driven individual who is passionate about making a difference to the care of the acutely unwell and deteriorating patients. 

Our CCOT is a proactive service that provides early recognition, assessment, and management of the deteriorating patient Trust wide.  As a CCOP, you are responsible for delivering patient-focused programmes of care, which address the complex and changing health and wellbeing needs of the deteriorating and critically ill patients throughout the Trust.

Providing autonomous specialist advice to support and facilitate high quality, individualised care to the acutely ill, deteriorating patients and their relatives/carers in collaboration with the multidisciplinary team.

Assisting and facilitating timely admission to the critical care area and involve the appropriate members of the multidisciplinary team in the decision-making process when determining escalation plans to prevent further deterioration.

Main duties of the job

The Critical Care Outreach Practitioner will work autonomously within all specialities across The Shrewsbury and Telford Hospitals NHS Trust and provide expert clinical practice and leadership on a 24-hour shift rotation that covers 365 days per year including bank holidays, providing expert care and intervention for the acutely ill, deteriorating and critically unwell ward patients across the Trust. 

Perform comprehensive patient assessments based on history taking and physical examination of the acutely unwell adult patients to arrive at a differential diagnosis. Undertake appropriate investigations and initiate, evaluate, and modify a range of interventions to prevent patient deterioration. 

Assist and facilitate in the timely admission to the critical care area and involve the appropriate members of the multidisciplinary team in the decision-making process when determining escalation plans to prevent further deterioration or inappropriate interventions. 

Respond to cardio-respiratory arrest and medical emergency calls with the skills to lead in emergency situations. Debrief teams if required and assist in the appropriate aftercare of the patient. 

Enable timely discharges for patients from the Critical Care Unit by supporting their continuing recovery and rehabilitation within the ward environment. 

Act as an independent non-medical prescriber.

Where you will experience a wide range of specialities.  At The Royal Shrewsbury Hospital there is predominately surgical and medical patients and at The Princess Royal Hospital, Telford you will experience medical, head and neck and maternity.

Qualifications

Essential criteria
  • Registered General Nurse (active NMC Registration)
  • Critical Care Course (HEE 6/7)
  • Physical/Health Assessment Course (HEE 6/7)
  • Independent Non-Medical prescriber (Nurse Independent Prescriber) (HEE 7)
  • Advanced Life Support Provider
Desirable criteria
  • Teaching in Healthcare/Clinical Education/Medical Education
  • Leadership Programme

Essential criteria
  • Proven experience and expertise of working within Critical Care Outreach or equivalent service
  • High level clinical skills and knowledge.
  • Experience of autonomous decision making.

  • Detailed experience of leadership, management and service development.
  • Involvement in Nursing audit and research.
