Job overview
A new opportunity has arisen at Shrewsbury and Telford NHS trust, to work with a friendly, compassionate and inspiring team as the departments Transitional Care and Outreach Lead, as part of investment this role is new to our department and is one of our key roles to the delivery of high quality and safe neonatal care.

Main duties of the job
You will be responsible for leading and managing our outreach nursing team and our transitional care. You will work across professional boundaries and manage a variety of nurses adult/child and midwifes. This role spans across community and secondary care services.
You will be required to build upon opportunities to further enhance family experience and to further integrate family integrated care, aiming where possible to keep family and baby together.
If role will require the candidate to undertake regular visits and provide clinical care to maintain competence and skill set both in the hospital and in the community setting, therefore car access and a full driving license is essential for this role.

Education and qualifications
Essential criteria
- Registered nurse/midwife
- Qualified in speciality or equivilent course
- Degree in relevant field
- Leadership training/qualification
- mentorship/assessment/training qualification
- Safeguarding children training
Desirable criteria
- Community/public health qualification/training
- UNICEF BFI training
- Early help training
Knowledge behaviours experience

- Proven experience neonatal band 6 and above
