Main duties of the job

In this varied and fulfilling specialist role, you will:

• ·       Work autonomously to assess palliative and end‑of‑life care patients, including history taking, holistic assessment, planning care and evaluating outcomes.
• ·       Provide expert advice and support to patients, families and multidisciplinary teams     
•         Lead and contribute to education for staff, promoting high standards of palliative and end‑of‑life care practice.
• ·       Support quality improvement initiatives, helping drive service development and best practice.
• ·       Collaborate withSpecialist Palliative Care Team and support the Lead Nurse in service delivery.
• ·       Work across adult clinical areas where patients with palliative needs are supported.
• ·       Participate in our 7‑day Specialist Palliative Care service, helping ensure consistent support.
• ·       Perform advanced clinical skills in assessment of palliative care or end of life care patients including history taking, holistic clinical assessment, advising on management plan and evaluation of care.
• ·       Put quality at the heart of practice by delivering evidence based individualised and person-centred care.
• Monitor and review the effectiveness of interventions with patientS & modify this to meet the changing needs and established goals of care.
• Prescribe and review medication for therapeutic effectiveness,  in accordance with evidence- based practice and national & practice protocols & legislation within scope of practice.
