Band 4
Job overview
This important role works as part of the RUH@Home multidisciplinary team, supporting adults to receive acute hospital-level care, rehabilitation and enablement in their usual place of residence.
You will manage a delegated caseload and deliver agreed physiotherapy and occupational therapy interventions within defined

For step-down patients already assessed by a registered therapist in hospital, you will undertake a structured follow-up assessment using the approved RUH@Home therapy assessment proforma. This will include reviewing mobility, transfers, function, rehabilitation goals, equipment, the home environment and relevant risks, with findings documented and complex or unexpected issues escalated to a registered therapist.
Please note: We are unable to offer Skilled Worker visa sponsorship for this role. To apply, you must already have an alternative right to work in the UK and will be required to provide accurate information about your current right to work status as part of the recruitment process.
Main duties of the job
In this important role, you will:
- Manage and prioritise a delegated caseload, working autonomously in patients’ homes within agreed competencies.
- Undertake protocol-led step-down assessments for patients previously assessed by a hospital Physiotherapist or Occupational Therapist, using the approved assessment proforma.
- Gather, interpret and record information on mobility, transfers, activities of daily living, rehabilitation needs, equipment, environmental factors and risk.
- Deliver and progress delegated rehabilitation, reablement and exercise programmes in line with agreed goals and treatment plans.
Monitor clinical and functional progress, recognise deterioration or variance from the expected pathway, and escalate promptly to the registered therapist or wider multidisciplinary team. - Support admission avoidance, timely discharge, prevention of deconditioning and reduction in ongoing care needs.
- Contribute to multidisciplinary handovers, care planning, discharge planning, service data collection and clinical documentation.
