Job overview
Lime is a all male acute mental health unit , the ward is looking for a motivated individual who has experience working with people with mental health issues , your role would be supporting patients on the unit looking at housing and blocks to discharge , you need to be confident leading on this subject whilst exploring and supporting a patient in the journey of recovery and discharge .
Main duties of the job
Assessing a patients housing needs and sign posting to appropriate organisations for support and unblocking barriers to discharge. This is a ward based job. Attending relevant meetings and assessments. Confident in written and verbal communication skills. Good computer skills. Identifying barriers on admission and working independently on these to prevent delayed discharge. Self direction and motivation is essential. Able to work in a challenging quick paced environment.

- EXPERIENCE IN MENTAL HEALTH AT LEAST 3 YEARS / OR RELEVEANT QUALIFICATION
- EXPERIENCE IN WORKING WITH PEOPLE AROUND HOUSING SOCIAL CARE
Desirable criteria
- CLEAR WRITTEN AND COMMUNICATION SKILLS
