We're an integrated community therapy team whose aim is to help the local population maximise their abilities to improve their quality of life. We have an emphasis on prevention, early intervention and long term self-management to enable patients to live better lives at home. Please note - this post would be suitable for a qualified OT or student due to qualify in 2024.

You will play an integral part of the Community Rehabilitation Team, managing a diverse community caseload including orthopaedics, frail elderly, post-surgery and neurological long-term conditions. You will have opportunities to support students and be involved in regular training and peer support.  We are part of an innovative neighbourhood model with close links with district nurses, mental health teams, community agents and adult social care staff as well as the rapid response and hospital at home teams. 

Main duties of the job

Assessment and treatment of adults with a rehabilitation need that can be met in the community. Maintaining peoples independence and reducing hospital admissions.
