Job overview
We are looking to recruit an experienced Physiotherapist to join our South Glos Later Life Therapies Team offering Physiotherapy to people in their own homes in the community receiving a mental health service from AWP, as well as contributing to the inpatient team on Aspen Ward, Callington Road Hospital. Referrals are primarily from the community mental health team for older adults, who offer a service to people over 65 as well as frailer younger adults or those with a dementia diagnosis. We also see people under the care of the Memory Service and the Specialised Care Home and Enhanced Dementia Service (SCHEDS).
This is a rewarding and varied role with opportunities for personal development and contributing to service development initiatives. You will work alongside an experienced Physiotherapy colleague and the wider Physiotherapy inpatient hub based at Callington Road Hospital. Support, networking and CPD opportunities are also offered across the wider Physiotherapy team trust-wide.
Main duties of the job
- Provide a physiotherapy service to identified service users under AWP care in South Gloucestershire Later Life service
- 1:1 in people’s homes.
- Walking aids and specialist equipment.
- Falls assessment and intervention plans.
- Balance, strength and conditioning.
- Safety assessments.
- Liaison with community and hospital physiotherapy and rehab teams.
- Contributing sessions to groups.
- Referral to community groups and community activity/sport/health resources.
- Advice around pain management.
- Co-morbidity of degenerative physical conditions and mental health.
- Carer advice and education.
- Lifestyle, wellbeing.
- Input and advice in care homes.
- Supporting physiotherapy students in team and role emerging placements.

Education and Qualification
Essential criteria
- Degree or Diploma in Physiotherapy
- Member of the Health Care Profession Council.
Desirable criteria

Experience and Knowledge
Essential criteria
- Significant demonstrable post registration experience functioning as a Band 5 practitioner
- Assessing and carrying out therapeutic interventions to the client group
- Evidence of continuing professional development.
- Knowledge of specific requirements to meet the needs of those with mental health conditions or learning disabilities.
- An understanding of multi professional team working and an ability to promote the role of the Physiotherapist within that.
- Knowledge of CPA (Care Program Approach) and how physiotherapy fits within process.
- Knowledge of relevant legislation within mental health and professional responsibilities.
- Awareness of the needs of individuals from diverse social, ethnic and cultural backgrounds
Desirable criteria
- Experience of working with clients in community settings.
- Experience working in a mental health or learning disabilities setting
Skills and Abilities
Essential criteria
- Clinical educator or willing to undertake training.
- Able to demonstrate the ability to teach and advise others.
- Be able to demonstrate the application of physiotherapeutic interventions in a variety of settings
- Highly developed communication skills – (both in writing and verbally) that are effective and in a manner appropriate to the individual and situation.
- Able to demonstrate highly effective clinical knowledge and apply it within the supervision process
- Able to show initiative within the parameters of effective team working and evidence based practice
- Able to contribute effectively to the strategic development of services
- Time management and ability to prioritise effectively
- IT skills , basic keyboard
