Job overview
As part of the ECHT (enhanced care home team), we are seeking a band 5 nurse to work as part of the discharge to assess pathway (D2A).
The D2A pathway is for patients that are medically fit to leave an acute hospital setting but not well enough to return to their previous place of residence. Patients are discharged through D2A pathway into a nursing home so they can continue receiving additional support and further assessment for a period of 28 days.
It will be the band 5’s responsibility to complete an initial physical health nursing assessment and follow up assessments of the patient in a timely manner. You will work closely with Therapy staff, IDT, social workers and care home staff to ensure that the safest outcome for the patient is met.
Main duties of the job
The Registered Nurse will:
- Be responsible for the assessment, development, implementation and evaluation of programmes for care home residents with support from Ward Manager / Team Leader / District Nursing Sister.
• Deputise, in the absence, for the Ward Manager / Team Leader / District Nursing Sister.
To work within a multi-disciplinary team delivering skills based care to patients in care homes across Doncaster. Working in partnership with other agencies to ensure delivery of a high quality service.

Essential criteria
- Current and active registration with NMC: Registered General Nurse
- Experience of working in a professional nursing role with patients with physical health needs Knowledge and experience of working in a multi-disciplinary setting. NHS experience Experience of working with patients who have long term conditions.
- Understanding of evidence based practice
- Complex reasoning, critical thinking, reflection, and analysis to inform assessments, clinical judgements, and decisions.
- Ability to work autonomously and within a team.
Desirable criteria
- Experience of liaising with staff across different disciplines and organisational boundaries.
Applicant requirements
You must have appropriate UK professional registration.
