Job overview

An exciting opportunity has arisen for a motivated individual to join our Community Cardiac 
Service within the Rotherham Doncaster and South Humber NHS Foundation Trust 
(RDASH). 
The postholder will undertake delegated clinical tasks, support patient monitoring and 
contribute to reducing avoidable hospital admissions in line with RDASH trust policies, 
procedures and values, working under the supervision of a registered nurse to support the 
delivery of safe, effective and compassionate cardiac care in the community, running clinics 
and home visits to support patients 

Main duties of the job

The main duties of the rule will include:

- Carrying out heart failure clinic reviews 

- Undertaking routine clinical observations including blood pressure, pulse, oxygen saturations, temperature following RDASH protocols. 

- Carrying out venepuncture. 

- Carrying out ECG recordings where trained and assessed as competent.

- Supporting patients with long term cardiac conditions, including heart failure, arrhythmia 

and coronary heart disease.

- Independently managing 24-hour tapes including the configuring and downloading of data. 

- Application of 24-hour heart monitors in a clinic/home base setting and utilising a self-fit 
option.

-Accurately report findings and any changes or concerns promptly to senior nurses.

-Monitoring of waiting lists to maintain a 4-week response time liaising closely with Admin 
and Clinical staff.

-Maintaining confidentiality in accordance with Trust policies and national guidelines.

Qualifications

Essential criteria
  • NVQ level 3 in Health and Social care or equivalent relevant experience
Desirable criteria
  • Access to nursing course

Experience

  • Experience working in a healthcare, community or care setting

Knowledge

Essential criteria
  • Understanding the importance of escalation
Desirable criteria
  • Knowledge of community health services

Skills/ Abilities

Essential criteria
  • Ability to carry out and accurately record clinical observations
  • Ability to work autonomously, on own initiative, and as part of a team.
  • Access to transport and the ability to travel within the required geographical area
  • Basic IT skills for electronic record keeping
Desirable criteria
  • ECG recording competency or willingness to undertake training
  • Obtaining venepuncture competency or willingness to undertake
  • Experience supporting patients with long term conditions.
