  • To support the continuity of care between community, multi-agency services and to ensure seamless care between specialised and non-specialised services.
  • The post holder will act as the first point of contact for children, young people and their families and for other professionals in Doncaster and will offer support and advice with the aim of minimising seizures and maximising patient and carers opportunity to live life as they wish.
