Rehabilitation Assistant

Full time 37.5 hours, Permanent Post.

Monday to Friday, with flexible working over seven days if required.

We are currently looking for an experienced, confident and enthusiastic Rehabilitation Assistant to join our Home First service. The current post would be based within the Specialist Falls Service however cross cover across the Home First Rehabilitation Services may be required at varying times.

The Home First Service includes Urgent Community Response, Community Physiotherapy and Occupational Therapy, Specialist Falls Service, Reablement and Intermediate Care Wards.

The Rehabilitation Assistant will support the qualified professionals and contribute to the assessment, implementation and delivery of care within patient’s homes, and rehabilitation exercise groups.

Candidates should be flexible, motivated and be able to develop the Home First service. We will provide support through in-house professional forums, Mandatory training, one-to-one and peer supervision.

Main Duties include:

  • To work with patient’s in the Doncaster area
  • To deliver rehabilitation programmes (including exercise programmes and Falls management programmes)
  • Educate individuals to self-manage their long term condition
  • Follow a plan of care as prescribed by the relevant professional
  • Provide mobility and compensatory equipment
  • Carry out the appropriate assessment and treatment with the patient
  • To work with patients on a one-to-one basis and in a group setting
  • Report back to qualified staff all findings and evaluations
