Job overview

The Home First Service comprises of Occupational Therapists, Physiotherapists, Nurses, Community Mental Health Nurses and Rehabilitation Assistants. The service covers the Inpatient rehabilitation wards, Urgent community response, Virtual ward service, Unplanned Physiotherapy, Occupational Therapy, Enhanced Care Home Team and Reablement.

 The Rehabilitation Assistant will support the qualified professionals and contribute to the assessment, implementation and delivery of care within the inpatient rehabilitation wards.

If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification [relevant to your role].

 As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.

An apprenticeship includes regular training with a college or other training organisation.  Around 20% of your working hours will be spent training or studying.  This leads to a nationally recognised qualification. 

Whether you already work for us or are joining the Trust and NHS for the first time, apprenticeships can help you to develop the skills, knowledge and know-how in order to embark on a successful and fulfilling career within the health service.

 

Main duties of the job

Assess the social needs of patients and their families / carers and identify and initiate appropriate steps for effective care linking in with health professional for advice if required.

 To work in daily partnership with other members of the MDT to ensure that patients receive a coordinated package of rehabilitation that is delivered in an efficient and timely manner taking into consideration their choices and preferences. This is includes attendance at MDT meetings and board rounds.

To be aware of new admissions to the unit within their working areas and ensure they participate actively in discharge planning

Qualifications

Essential criteria
  • English and Maths at level 2 or equivalent (GCSE 9-4 or A*- C or functional skills level 2)
  • NVQ Level 2 in a related subject
  • NVQ level 3 in related subject or willingness to undertake a relevant Level 3 Apprenticeship

Experience

Essential criteria
  • Demonstrate a keen interest caring for others
  • Previous experience of caring for patients in a clinical setting
  • Previous experience of working as part of a team

Knowledge

Essential criteria
  • Understand importance of maintaining confidentiality
