Job overview
The Memory Service is offering an exciting opportunity for one year to develop and progress as a health care assistant. This is a fast paced out patient department for people with cognitive decline offering initial assessment, diagnosis, treatment and educational interventions to enable and empower a person to live the best life they can with a diagnosis of dementia.
Main duties of the job
You will need the skill to be able to engage with our service users, have excellent written and verbal communication skills and be an active listener.
You will work closely, supporting the qualified team of nurses, occupational therapist, psychologist and medics, delivering interventions and supporting written care plans.
You will need to have the ability to use your own initiative and to work in challenging and changing situations and be willing to learn and engage with new and exciting developments
You will need to be confident using IT skills and have a working knowledge of the RIO system.
You need to be working within the Trust values at all times
A full clean driving license is required for this role

Detailed job description and main responsibilities
- To perform duties under the direction and indirect supervision of a named qualified professional
- To recognise when to seek support or escalate a clients needs or risks to qualified team members
- To undertake ECG, Pulse, Blood Pressure, Weight with patient either in hub or preferred community setting
- To provide a follow up service either face to face, virtually or by phone and record accurately in electronic care records
- To maintain positive relationships within the multidisciplinary team and all others involved in the care of the service users
- To arrange/attend discussions regarding service user care as and when required and provide input into the evaluation of the collaborative care plan
- To build therapeutic relationships with the service users/supporters and provide reassurance and information regarding the care pathway
- To facilitate/co facilitate psychosocial and educational interventions with service users and their supporters
- To encourage and enable service users to be fully involved and engaged with their agreed care plan and to access appropriate services
- To promote equality for all individuals by promoting and supporting individuals rights and choice
- To take a role in health promotion with service users and their supporters
- To organise their time effectively
- Tow work flexibly to service requirements
- To utilise and familiarise themselves with the Trusts computer systems and keep updated with new inititiatives
- To organise referral on to other services as required
- To be aware of risks associated with being a lone worker in a community setting, following the lone worker policy
- To maintain records of activity, mileage incurred and relevant statistical information as per standard operational procedures and policies
- To attend relevant training programmes and updates for dementia and associated conditions

- Maths and English GCSE or equivalent (e.g. functional skills; ENIC)
- Vocational qualification in health and social care (e.g. care certificate; level 2/3 diploma)
- Working knowledge of RIO system with good IT skills
Desirable criteria
- knowledge of dementia medications e.g cognitive enhancers
- knowledge of mental capacity act level 1
- knowledge of DOLS level 1
Experience
Essential criteria
- Experience of working with people who have dementia
- Experience of providing person centred care
Desirable criteria
- Experience of working in the community
- ability to speak to & reassure relatives & carers
- ability to help a person complete a form
- a basic understanding of psychosocial intervetions e.g. anxiety management, mindfulness
Knowledge and skills
Essential criteria
- Good written and IT skills
- Knowledge of how to do pulse, BP
- Ability to organise workload & work with own initiative
- Knowledge of therapeutic activities for older adults
- Experience & knowledge of how to de-escalate a situation and remain calm during the crisis
Desirable criteria
