Senior Occupational Therapist Adult Community Mental Health

Are you looking for a new role utilising your Occupational Therapist skills within the community Mental Health teams?  We are seeking 2 enthusiastic  band 6 Occupational Therapists , full time or part time opportunities available to join the Community Rehabilitation Team in North Lincolnshire, Scunthorpe

The core hours are 37.5 per week, 7 days a week – flexibility of the daily hours can be agreed.

We have an exciting opportunity to join a new mental health community rehabilitation service for people living in North Lincolnshire and we are looking for enthusiastic and innovative professionals to help us embed this new 7-day service.  The service will enable people to receive an intensive community rehabilitation service closer to home in their own community.  We are transforming with the NHS Long Term Plan (2019) for Mental Health at the heart of our innovations.

 The community rehabilitation service will consist of a multi-disciplinary, multi-agency workforce, working collaboratively between Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) and North Lincolnshire Council to share skills and specialisms to support individuals and improve their quality of life.

MAIN DUTIES

·       To work within an occupational therapy mental health pathway aimed at supporting people to achieve their recovery goals/ functional stability

·       To carry out assessments and interventions, utilising the OT process, from initial assessment to discharge planning and evaluation. Develop clinical formulation and apply clinical reasoning.

·       To undertake risk assessments; document, report and act on identified concerns.

·       Treat, monitor, develop each individuals OT programme, measure outcomes and discharge appropriately.

·       To use MOHO as a core framework for practice (including MOHO based standardised assessments and formulations for pre-post therapy outcome measures and patient-centred goal planning)

·   Educate, guide and supervise patient as individuals or in groups.  

·  Educate carers on the support and management of the patients needs.  

· Negotiate, agree expectations and set appropriate, realistic and individualised therapy goals in partnership with the patients, carer and colleagues. 

·  Participate in multidisciplinary case review meetings to discuss progress and to plan treatment and safe discharge.

·       Refer to other agencies as appropriate.

· Communicate complex, sensitive, distressing and contentious information effectively and compassionately with patient, carers, and staff in the multi-disciplinary team and other agencies.
