Job overview

The Care Home Liaison Nurse plays a key role in delivering high-quality, person-centred care to residents in care homes. They act as a specialist resource to care home staff, patients, families, and primary care teams, aiming to prevent unnecessary hospital admissions, promote early intervention, and support safe and timely discharges from acute settings. The role involves clinical assessment, care planning, training and collaborative working across multidisciplinary teams.

Main duties of the job

·       To provide specialist support and advice to referring Care Homes, preventing unnecessary delays.

·       To promote and assist in the maintenance of the highest attainable standard of nursing care in a Care Home setting, drawing on examples of current best practice.

·       To deliver education, support interventions, and implement processes with consequent improved patient outcomes, therefore supporting the achievements of Rotherham Doncaster and South Humber NHS Foundation Trust and Rotherham Foundation Trust internal performance indicators.

·       To undertake clinical effectiveness and audit activities throughout the year.

·       To promote and assist in effective ward discharge and placement preparation for both the patient and their family/carer.

·       To ensure Mental Health Act and Legislation advice is provided to Care Homes as required.

·       To review, discuss and monitor medications liaising with GP and Psychiatrists.

·       Provide support and advice in relation to patients/family/carers with memory problems and liaise with specialist service if required to commence medications.

·       To carry out Section 117 reviews and liaise with appropriate others.

·       To liaise effectively with Inpatient ward, Community teams, Care Home Manager and staff, assisting in safe and effective care planning and supporting with transition of Care.
