Job overview

The  Primary care routine pathway within the Comprehensive Emotional Care Mental Health Team has an exciting opportunity for an experienced  adult mental health nurse to join our team.

This role will involve offering triage, assessments, reviews and interventions for patients referred to the team, in accessible locations across North Lincolnshire. Networking between Primary Care and Secondary Care Services is a central part of the role to ensure the patients journey between services is seamless and care and treatment is provided in a timely manner.  The role will also involve working with the wider team to triage and offer routine mental health assessments to  adults experiencing mental health problems within Primary Care.

Main duties of the job

As a member of the  Team the post holder will provide high quality nursing assessment and interventions within the primary care routine Pathway.

The post holder will be an enthusiastic, highly organised and motivated mental health professional who can provide high quality nursing expertise and support to patients.

It is essential that the applicant can demonstrate post registration experience as an RMN, which may include inpatient and community settings. 

The following tasks and duties will also form part of the role, however this list is not exhaustive:

  • Triage of  incoming referrals to establish urgency / risk.
  • Holistic mental health assessment.
  • Development of appropriate treatment plans / signposting to relevant third sector organisations.
  • Recruitment, supervision, and deployment of staff.
  • Advice, training and induction and mentorship of learners.
  • Audit, research and service monitoring.
  • Ordering, storage, monitoring and administration of medication.
  • Liaison within the multi-disciplinary care team, including carers, voluntary agencies etc.

Essential criteria
  • Registered Mental Health
  • Nurse Willingness to undertake additional training
  • Evidence of post registration training
Desirable criteria
  • Counselling Qualification
  • Venepuncture Qualification

Experience

Essential criteria
  • Demonstrable post registration experience as an RMN
  • Experience of MDT working
  • Skills in coordinating and planning interagency approaches in mental health care.
  • Experience of assessing and managing risk
Desirable criteria
  • Experience of working in the community
  • Experience of working with statutory and non- statutory agencies.
  • Experience of working at Band 6 level
  • Experience of managing risk.
  • Experience of delivering evidence-based treatments
  • Experience of cognitive testing

Knowledge and skills

Essential criteria
  • Ability to use initiative and work without immediate supervision
  • Knowledge of cognitive testing processes.
  • Knowledge and understanding of the legislative framework for mental health services, particularly the Mental Health Skills in CBT Ability to work in a changing environment Knowledge and understanding of the roles of other professionals working within mental health A/I Act, Mental Capacity Act & NICE Guidelines.
  • Ability to work as part of a multiprofessional team
  • Ability to demonstrate an understanding, empathic approach
  • Skills in co-ordinating and planning interagency approaches in mental health care
  • An understanding of current mental health issues
  • An understanding of a range of mental health diagnoses and associated treatment options and nursing interventions required
  • Excellent report writing skills
  • Excellent assessment skills communication skills, establishing a rapport, focusing on gathering pertinent information, probing questions and ability to assimilate within your mental health knowledge to agree an appropriate outcome and plan.
  • Completing robust, initial risk assessments and ability to review historic risk.
  • Understanding of the importance of user/carer involvement in care planning
Desirable criteria
  • Skills in CBT.
  • Ability to work in a changing Environment.
  • Knowledge and understanding of the roles of other professionals working within mental health.
  • Ability to elaborate on the translation of personal and professional skills into practice.
  • Understanding of Dialogue.
