The Specialist Community Public Health Nurse (SCPHN) will work as part of an integrated team contributing to family health needs assessment. The post holder will lead the 0-5 Healthy Child Programme and work in partnership with the family, education and other agencies to develop implement and evaluate packages of care. The health Visitor role forms a vital and integral part of the 0-19 offer.

Main duties of the job

Clinical Nursing Practice

Deliver the commissioned service

Utilise specialist skills and knowledge to assess the health and health related needs of children and their families. Identify, develop, deliver and review packages of care that ensure effective treatment and referral to maximise potential health outcomes.

Work as an autonomous practitioner, assessing and delivering care in a range of settings, assuming professional and legal accountability for all aspects of own work in line with the Trusts objectives and national and local guidance.

Ensure adherence to nurse prescribing policy when utilising specialist nurse prescriber knowledge to diagnose, prescribe and treat a range of clinical conditions and ensure adherence to nurse prescribing policy.

Undertake child health promotion, utilising therapeutic relationships with individuals, families and communities

Identify and minimise risk to ensure children are protected and remain safe, working in partnership with families and other agencies.

To complete accurate, contemporaneous records on electronic record systems in a timely manner in line with NMC and Trust guidelines and policies.

To provide up to date evidenced based care, advice and support to children and their families.

  • A Specialist Community Public Health Nurse (Health Visitor) with an active registration
  • RGN, RN or RM

Experience

  • Appropriate demonstrable experience aligning to the advertised job role.

Knowledge

  • Employ the principles of Community Specialist Public Health Nursing (HV)
  • Identification and evaluation of the best evidenced based practice in order to deliver effective health care intervention
