We are a knowledgeable, motivated and supportive team looking for enthusiastic individuals. 

Our aim it to ensure that people experiencing a mental health crisis are offered assessment and intervention quickly and effectively. 

 

Main duties of the job

  • To triage urgent/crisis mental health referrals from a variety of sources and to establish an appropriate response.
  • To offer mental health assessment, formulation and time limited treatment planning for individuals needing an urgent/crisis response

 

  • To provide a resource of highly specialized, time limited interventions to individuals for up to 72 hours following assessment.
  • To work with other disciplines and teams within Sheffield Health and Social Care (SHSC) to ensure service users benefit from a multi-disciplinary approach that best matches clinical need.

 

  • To develop and maintain close working relationships with existing mental health teams, primary care services and other key stakeholders.

 

  • To promote the Urgent & Crisis Service through participation in local and national arena.
    • To work flexibly to develop team working and to ensure consistent standards of service delivery throughout wider services.

     

    • To actively participate in developing team function through tests of change, measuring outcomes, audit and service evaluation.

     

    • To work in co-production with service users, providing a high quality and flexible service to meet individual’s needs.

     

    • To develop strong links and protocols with other services to ensure smooth and timely transitions for patients moving between services

     

    • To include families / carers in decision making and planning

Detailed job description and main responsibilities

  • To undertake the triaging of mental health referrals meeting the threshold for urgent/crisis service and to establish an appropriate response.

 

  • To undertake mental health Crisis assessments, formulation and time limited treatment planning for individuals needing an urgent/crisis response

 

  • To conduct psychosocial mental health and risk assessments in accordance with service policy

 

 

  • To develop close working links with other mental health teams and agencies to enable swift and appropriate transfer between services.

 

  • To contribute to and deliver time limited, co-productive care plans in consultation with service users, families/carers, community teams, primary care and other appropriate service providers.

 

  • To provide comprehensive written reports as required.

 

  • To participate in clinical supervision both on an individual basis and team/peer supervision. Also to engage in delivering supervision to more junior members of the team.

 

  • Maintain clinical records according to professional and Trust standards.

 

  • To attend and contribute to clinical meetings within SHSC.

 

  • To work flexibly and provide cover in the absence of other clinicians within urgent & crisis service.

 

  • Ensure efficient recording of activity and outcome data.

 

  • To actively take part in and encourage the placement of students within the service.

Essential criteria
  • RNMH /Registered Social Worker having current registration with the relevant regulatory body.
  • Evidence of continuous professional development in mental health nursing/social work
  • Evidence of higher practice skills in mental health care across the age range
Desirable criteria
  • Teaching/Assessing qualification or equivalent.

Knowledge and Skills

Essential criteria
  • To possess excellent risk assessment and management skills for service users experiencing urgent/Crisis mental health challenges.
  • Ability to assess plan, implement and evaluate care for service users within urgent/crisis mental health services.
  • The ability to work and make decisions autonomously.
  • Knowledge and understanding of MHA , MCA other relevant legislation.
  • Knowledge and experience of relevant mental health treatment modalities.
Desirable criteria
  • Experience of crisis assessment/ formulation work
  • Skills in team working
  • Awareness of other agencies within Wider Community.

Experience

Essential criteria
  • Currently employed as a Band 6 Mental Health Nurse or Registered Social worker.
  • At least 12 months experience (following preceptorship) as a Band 5 Mental health Nurse/ Level 1 Social worker having completed ASYE
  • Experience of working in acute mental health care settings.
  • Active involvement in promoting Clinical Supervision
Desirable criteria
  • Demonstrable experience of collaborative working within a multi-professional environment.

Essential criteria
  • Ability to work autonomously, Confidence in working in isolation, as well as part of a team
  • Ability to motivate and inspire confidence in others
  • Balance issues of safety and user preferences
  • Adherence to the Code of Professional Conduct
  • Able to work Monday to Friday between the hours of 8am-8pm, working 7.5 hours a day
