Palliative Health Care Support Worker

Job overview
We are looking for Band 3 health care support workers to join our established team to deliver Palliative and End of Life Care for patients within the community.
The primary objectives of the service is to work closely with community services within Midlands Partnership Foundation Trust, charitable and voluntary services, and care agencies to address a wide range of care/support issues to improve end of life care for patients, their families and carers.
You must have the ability to work as part of a team and to be able to work alone. Candidates must be able to demonstrate an understanding of palliative care and possess the dedication and commitment to supporting and caring for individuals, their families and carers.
Staff must have their own transport for work purposes. Full driving licence and access to a vehicle is essential
Specific training will be provided to support staff development and ensure a quality service delivery.
Shifts will comprise of 8am-4pm / 10am-6pm, with staff being expected to work flexibly in response to service needs. Your base will be at Norton Canes Health Centre (Cannock, Staffordshire, WS11 9SE) and will predominately be supporting patients in the South, East and West of Staffordshire. There will be a requirement to visit the main base in North Staffordshire. (Oak Ward, Bradwell Hospital, Newcastle U Lyme, ST5 7NJ)

Main duties of the job
· Be involved in the first point of contact in the care pathway
· Assist in the delivery of outcome based goals and self-directed goal planning
· Undertake person centred support planning
· Deliver support plans and safeguard vulnerable people
· Contribute to outcome focused reviews
· Encourage peoples independence, choice, health & wellbeing
· Encourage individuals use of equipment and assistive technology
· Facilitate individuals transition to alternative services and support if needed
· Engage in organisational learning and development
· Provide excellent patient care & support

- Care Certificate Level 3 NVQ or Diploma in Health and Social Care or Equivalent level qualification
Experience
Essential criteria
- Experience of providing care in a formal or information setting equivalent to NVQ level 3
- Be person centred; undertake care and support activities in a way that reflects the preferences and independence of people
- Understanding of people with palliative care needs
Desirable criteria
- Work in a community based health and social care service
- Supporting people with palliative care needs at the end of their lives
Skills Knowledge & Abilities
Essential criteria
- Able to work as part of a team, Excellent communications skills using a variety of communication mediums
