Job overview

A rare and exciting opportunity has arisen for an enthusiastic and self-motivated individual to join the South Staffordshire Hospital Discharge Team based at Great Wyrley Health Centre.

The role of the Discharge Liaison Sister / Charge Nurse will be based in our Triage hub, providing clinical expertise to our referring acute hospital trusts. The main referring hospitals in the South of the county are Russell’s Hall Hospital, Newcross Hospital, Walsall Manor Hospital, Good Hope Hospital and Queens Hospital Burton. 

Main duties of the job

The successful candidate will be part of a triage team that receive referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients’ needs as part of a multi-disciplinary team with a key role in decision making.  Part of the role of the Discharge Liaison Sister / Charge Nurse will be to support end of life patients to receive the best quality care in the place of choice determined by the patient. 

The post holder will liaise with a variety of people (service users, colleagues, member of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual’s ability/potential for reablement via the discharge to assess pathways alongside supporting admission avoidance assessments to prevent a hospital admission.

Detailed job description and main responsibilities

The post holder will be working autonomously as part of a hub and spoke model and will be expected to be able to use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams.

The service currently operates a 7 day a week service through the hours of 8am to 8pm including weekend and bank holidays. This would be negotiated on a pro rata basis.  

We are looking for a Nurse that has a passion for making a difference to our patients, someone who wants to enable individuals to live their best lives through the use of our reablement service. We are also looking for an enthusiastic and innovative individual who possesses effective telephone and communication skills, IT skills, and exceptional organisational skills.

Essential criteria
  • • Registered General or Mental Health Nurse with significant post qualification experience.
  • • A wide range of significant clinical experience including hospital and community settings at a senior staff nurse level.
Desirable criteria
  • • Ability to plan, organise, review wide range complex activities and to provide and operational lead in their internal implementation.
  • • Experience of multi-agency and multi-disciplinary working.

  • • Ability to travel independently to include working between several different locations on a rotational basis including potential on-call duties.
