Are you an experienced Senior Nurse looking for an opportunity to utilise your proven leadership skills alongside patient clinical assessment and acute care delivery? If so, this unique role is for you.
The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, to provide clinical assessment, monitoring and support, as part of our urgent care pathways.
The post holder is required to hold a non-medical/supplementary prescribing qualification. However, applications will also be considered from clinically experienced applicants with the relevant skillset currently undertaking the course or willing to undertake on appointment within the first 12 months.
Main duties of the job
The post holder will be an experienced Acute Care nurse with proven team leadership skills and clinical competence to deliver direct care to a wide range of patients. They will incorporate independent non-medical prescribing, physical health assessment, diagnostic testing and interpretation. They will formulate tailored treatment plans in conjunction with the Ward Consultant, to provide safe, seamless care to patients recovering from a wide range of illnesses in a community setting.
They will act as the shift lead working closely with the Matrons and nurse coordinators, acting as a professional role model to provide clinical leadership, support and guidance to the Virtual Ward team and ensuring full clinical oversight of quality and safety.
They will initiate, manage and drive change in practice for the benefit of patient care and service development in line with local and national policy and the NHS People Plan. They will take an active role to ensure that evidence-based practice is maintained to deliver the Virtual ward agenda supporting earlier discharge from hospital and admission avoidance.
In addition, they will hold delegated managerial responsibilities under the supervision of the Matrons and provide ongoing support to junior members of the team, participating in induction and training programmes.

Please note that this role requires the postholder to have access to a vehicle which can be used for work purposes or able to travel as required to meet the needs of the service. Please note not all office locations or client care bases may be reached by public transport.
