We are offering an exciting opportunity to join our Older Persons and Physical Disability Teams as a Level 2 Registered Social Worker based in the Staffordshire Moorlands.

The service supports people of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

Are looking for a highly skilled professional with excellent communication and organisational skills with an excellent working knowledge of the Care Act 2014 and Mental Capacity Act 2005.

Main duties of the job

As a Level 2 Social Worker, you will offer a flexible approach with the ability to forge excellent working relationships with multi-disciplinary teams and partner agencies.  You will be expected to be an experienced practitioner with in-depth knowledge of safeguarding and complex case management.  

As part of this role, you will be expected to:

·       Play a key role to play in supporting the delivery of the Personalisation and Inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset based practice, personalisation and social inclusion is embedded and applied effectively.

·       Carry out a range of assessments including holistic needs led assessments, work of a more complex nature, care and support planning and reviews, and, where appropriate, initiate a multi-disciplinary assessment.

·       Have comprehensive knowledge of the services available to support and promote independence in the home environment is essential.  

·       Providing strength-based practice, forging links within local communities to provide an outcome focused, person centred service as we move towards a meaningful integrated care system.

On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework.  The relevant SWLA ‘stage’ (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Person specification

Qualifications & Training

Essential criteria
  • Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
  • Ensuring current registration with the Professional Social Work regulatory body at all times.
  • To hold/to undertake and achieve the status of Approved Mental Health Professional and/or Best Interest Assessor as per the SWLA Career Pathway
  • To undertake and achieve the status of Work Based Supervisor and Practice Educator as per the SWLA Career Pathway
  • Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
  • An ongoing commitment to personal and professional development.
  • A clear understanding of and commitment to equal opportunities and anti-discriminatory practice
Desirable criteria
  • Existing accreditation relating to Practice Educator/Work Based Supervisor

Experience

Essential criteria
  • Significant post-qualification experience of working with adults with needs associated to older age, physical health or mental health needs and their carers
Desirable criteria
  • Experience of social work in other user groups/settings i.e. Children and Families, Learning Disabilities, Older Adults Experience of a development role for service/ facilities
