Main duties of the job

As a Community Staff Nurse you will deliver a high quality service, working closely with GP’s and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions.

You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.

Post holders will be required to work flexibly on a rota system across a 7 day working week.

  • To be competent as appropriate to the requirement of the role. 
  • Demonstrate at all times the Partnership Trust Values of 'We Put Quality First, We Focus on People, We Take Responsibility'. 
  • Be a Staff Nurse and have a professional and clinical knowledge of their discipline. They will have a focus in one of the following areas; 
  • Delivery of care to patients who are in their own homes or residential homes, and or delivery of care to patients within a clinic setting where these are available and appropriate. Be responsible for a defined population of service users in a variety of settings across the Partnership Trust, including the community and hospitals. 
  • Be skilled in the assessment of care/support needs and in development, implementation and evaluation of care/support packages. 
  • Have supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.
