Job overview
The Community Mental Health Rehabilitation Service is a new and developing service that works with people with complex severe and ensuring mental illness (psychotic illnesses) and their families within their local communities.
The service will provide specialist assessment, treatment and support to stabilise people’s symptoms and help them gain/regain their skills and confidence to live successfully in the community – adopting a team approach to care delivery.
The service will support individuals in their own homes in the community, in supported accommodation and those returning from out of area hospitals.
The service has strong working relationships with other services both internally and externally, striving to make a significant difference in our communities.
We promote regular CPD opportunities both internally and externally and support individuals to develop specialised nursing skills to work in a complex area of mental health care.
Main duties of the job
- Undertake clinical assessments to establish the person’s needs, goals and risks and generate a formulation based care plan with all involved.
- Deliver recovery focussed and evidence based interventions, in accordance with an agreed care plan, to an identified group of people
- Manage a caseload, as named keyworker, of individuals with complex severe and enduring mental illness
- Administer medication, complying with relevant Trust procedures and NMC Guidance.
- Complete and contribute to robust risk assessments and risk management plans - deliver and promote positive risk taking
- Participate in the development of the service - assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.
- Possess excellent communication and interpersonal skills, and have an ability to build positive multidisciplinary team relationships, and work collaboratively with external agencies, patients and their families/carers.

Detailed job description and main responsibilities
- Take responsibility for the management of a complex caseload, including specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Review Process).
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
- Administer medication, complying with relevant Trust procedures and NMC Guidance.
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
- Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
- Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
- Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
- Assist in ensuring that the aims and objectives of the service (as set by the operational lead) are fulfilled and to identify factors which may inhibit these from being achieved.
- The post holder will be required to work within the NMC Code of Practice and professional guidelines.
- Maintain an up-to-date level of professional competence within the specific environment.
- Assist in the implementation of evidence-based nursing practice and participate in research and other projects as required.

Qualifications
Essential criteria
- Registered mental health Nurse on NMC Register.
- Post-registration experience, evidence of formal courses and training at diploma/degree level.
Desirable criteria
- Leadership qualification/course
- Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT
- Registered Mentor
Experience
Essential criteria
- Evidence of post registration experience working with adults experiencing complex severe and enduring mental health needs
- Experience of working with people who have difficulty in engaging with services and have complex mental health needs
- Experience of working in community setting and making decisions autonomously
- Experience of assessing, planning, co-ordinating and managing people with complex severe and enduring mental health problems in a community setting
Desirable criteria
- Experience of integrating care delivery across multi-disciplinary /multi agency teams
Skills
Essential criteria
- Knowledge of the Mental Health NICE Guidelines and Long Term Plan
- Knowledge of NMC Code of Conduct
- Knowledge of other relevant local and national documents/drivers that affects practice
- Knowledge of the wide range of statutory independent and third sector service provision for adults with mental ill health
- Knowledge of pharmacological interventions prescribed in mental health
- Knowledge of Mental Health Act, the Mental Capacity Act and DOLS
Desirable criteria
- Skilled in utilising specific assessments and outcome measures including; GAD7, PHQ9, CAARMS, PANNS, SWEMWBS, Lunsers, GASS
- Knowledge and experience of holistic assessment of someone’s mental ill health, strengths and needs and appropriate nursing and multi-disciplinary and agency interventions.
- Skilled in undertaking and interpreting physical health investigations and providing physical health promotion advice including: weight, BMI, blood pressure, smoking, alcohol, drugs (prescribed and non-prescribed)
- Supervisory skills and knowledge to be able to provide clinical supervision
