Are you a Mental Health Nurse who wants to make a difference to older adult care in Shropshire?
We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Nurse to join our Dementia/Older Adult Community Mental Health Services within the Shropshire, Telford and Wrekin locality.
This is an exciting opportunity to be part of a newly developing, dynamic and supportive multi-disciplinary team delivering an enhanced offer in partnership with colleagues to older adults who are experiencing an organic and/or functional mental health needs. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.
Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;
- Provide specialist assessment and care planning in conjunction with wider dementia and community services, service users and carers
- Evaluate treatment plans
- Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
- Co-ordinate the work of the Team
Main duties of the job
- Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
- Administer medication, complying with relevant Trust procedures and NMC Guidance
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.
- Deliver a range of interventions in accordance with the agreed plan of care and pathway guidance including; pharmacological and psychologically informed interventions.
- Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

Skills
Essential criteria
- Skills of assessing, planning and evaluating care, for people with mental health and dementia related difficulties, utilising the principles of Recovery
- Knowledge of the physical health needs and monitoring requirements for older people receiving community mental health and/or dementia services
- Knowledge of Mental Health Act and the Mental Capacity Act
Qualifications and Training
Essential criteria
- Registered Mental Health Nurse on NMC Register.
- Post-registration experience, evidence of formal courses and training at diploma/degree level.
- Registered Mentor
Desirable criteria
- Qualification in psychological interventions such as DBT, SCM, CBT, EMDR, MBT, relevant to a client group with complex mental health needs
Experience
Essential criteria
- Evidence of extensive post registration experience with working with older adults experiencing dementia and/or functional mental health illnesses
- Experience of working with people who have difficulty in engaging with services and have complex co-morbidity needs
- Experience of working in community setting and making decisions autonomously
- Experience of integrating care delivery across multi-disciplinary /multi agency teams
- Experience of leading/contributing to care planning processes
