Job overview

This is an exciting opportunity to work as part of the existing Health & Justice Outreach Team on a 12-month Fixed term contract. The Pilot project is to expand the existing service to provide a Voluntary Attendance pathfinder across Staffordshire and Stoke on Trent. This pilot will increase the capacity of the Health & Justice team to have the resource to respond to individuals that enter the criminal justice system via a voluntary attendance. 
 The service will operate a Hub that will be operational Monday to Friday, 9am to 5pm receiving referrals from Staffordshire police for individuals invited for a voluntary attendance interview.
The fixed term post will increase the capacity of the existing Health & Justice Outreach team to ensure that the Hub is operational Monday – Friday.

This role will include:
•    Independently undertake face to face screening to support the identification of multiple vulnerabilities. 
•    Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.
•    Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.
•    Liaise with other agencies as required dependant on the person’s presentation, work closely with other agencies.

•    Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system.
•    Work within limitations and escalate to Practitioner as appropriate.
•    To support Practitioners, STR Workers and Lived Experience workers and Admin as necessary.
•    Provide an embedded service in police custody suites, courts and Outreach, working within legislation requirements, such as PACE (1984).
•    Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.
•    Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.
•    Liaise with other agencies as required dependant on the person’s presentation, work closely with other agencies, such Custody and Health Care Professionals, and community agencies.
•    Actively promote the service to external agencies as a positive example of user led services.

Detailed job description and main responsibilities

Working closely with the multi-agency Health & Justice team, you will receive referrals for individuals in the Health & Justice population and you will.
•    Triage referrals
•    Assess service users’ individual needs.
•    Complete and review risk assessments and risk management plans
•    Offer liaison, joint-working and referrals to other relevant services
•    Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, such as police custody, courts, and community settings. Working to guidelines.
•    Work within limitations and escalate to Practitioners as appropriate.
•    To support Practitioners, STR Workers and Lived Experience workers and Admin as necessary.
•    Provide an embedded service in police custody suites, courts working within legislation requirements, such as PACE (1984).

Qualifications

Essential criteria
  • NVQ Level 3 Health & Social Care (or willingness to work towards) or equivalent experience.
  • The post holder will require additional vetting by partner agencies; this includes Police Vetting and / or Prison Clearance.
Desirable criteria
  • Training in Supporting / Mentoring / coaching.

Experience

Essential criteria
  • Experience of working with a range of vulnerabilities which may include mental health, physical health, learning disabilities, abuse, addiction and social issues.
  • Experience of working with a range of statutory, non-statutory agencies.
Desirable criteria
  • Lived Experience of vulnerability, criminal justice systems and / or leaving care.
  • Experience and/or sound knowledge of the Criminal Justice System.
  • Experience of working in a community based setting.
  • Experience of implementing personalised care plans or rehabilitation plans.

Skills, Knowledge & Abilities

Essential criteria
  • Awareness of a range of vulnerabilities. An awareness of the impact of vulnerabilities on individuals.
  • Basic understanding of the Recovery Model.
  • Ability to form rapport with individuals and groups.
  • A basic understanding of the Mental Health Act 1983(07).
  • Ability to maintain a high degree of professionalism at all times, even in challenging and complex situation.
  • Excellent typing skills and ability to type with precision and speed.
  • Effective organisation / planning skills.
  • Willingness to work flexibly.
  • Ability to travel across the MPFT footprint.
