Job overview

North Staffordshire Combined Healthcare are looking to recruit a Band 5 Community Mental Health Practitioner [Nurse] to support the City CMHT Team at the Sutherland Centre.

The post is full time and the successful candidate will ensure the mental health needs of an identified group of patients are met.

you will contribute to the assessment, development and  delivery of evidence based care pathways and work on the Triage and duty team.  You will be responsible for the assessment of needs and the planning, delivery and evaluation of care and to deliver specific interventions.

Main duties of the job

  • To assume responsibility and accountability for patients requiring duty support, triage and undertaking team assessments. To ensure that each has a current, individualised and comprehensive care plan based upon assessed need.
  • To carry out all care and associated tasks for supporting patients without direct supervision.
  • Supervise junior/unqualified staff in the provision of care.
  • To contribute to the day-to-day operation of the clinical area in respect of delegated administrative procedural tasks.
  • To support senior staff in the planning and implementing of new initiatives.

Detailed job description and main responsibilities

  • To report to the person in charge/clinical manager any matters that are likely to affect the provision of patient care within the area.
  • Monitor the physical environment with due regard for standards in relation to hygiene and safety. Maintain a safe and orderly environment, identify potential hazards and where possible take appropriate action, if necessary reporting to the person in charge and/or liaise with support services staff.  Promote the active involvement of the patient group (where appropriate) in maintaining a clean and orderly environment.
  • Ensure that resources are utilised effectively and stored correctly. Participate in auditing or requisitioning procedures acting on any deficiencies as appropriate.
  • To occasionally take charge of the clinical area in the absence of a senior manager. (An average of once per week)  To have responsibility for the management of the clinical area, the prioritisation of tasks and supervision of junior staff during this time.
