Main duties of the job

An exciting and interesting opportunity has arisen for an ambitious and enthusiastic Lead Practitioner to work within the South East & East LAP/CCC 

We are looking for a compassionate and motivated experienced Nurse who has a high level of clinical expertise and leadership skills. The post holder will need to demonstrate a specialist interest in the specialism of Triage and hold a comprehensive knowledge base, promote excellence in nursing practice in a dynamic healthcare environment. 

They will be responsible for the professional development and  clinical supervision of Triage nurses and the delivery of a 7 day consistently high quality, safe and sustainable, cost-efficient service.

Detailed job description and main responsibilities

To providing clinical leadership to the Triage nurses and Administration staff within the Local Access Point/ Care Coordination Centre. Supporting the team to ensure safe and effective care for service users. 

Working alongside the Operational Manager to support the day to day running of the Team and development of staff within the triage team.

To provide a comprehensive triage assessment to service users involving  complex decision making to ensure that service users access the appropriate service in a timely manner.  

To be experienced in a range of nursing procedures including but not limited to wound care; end of life care; continence management; administration of medications through various routes including I.Vs.

To act as a role model and a senior member of the Local Access Point sharing clinical knowledge, expertise and advice and provide mentorship and supervision to pre and post-registration nurses.

To ensuring robust escalation and business continuity plans are in place and delivered during periods of high demand and/or reduced capacity.

To working closely with other MPFT services and external partners to support the development and delivery of efficient and effective care pathways. Supporting the delivery of place based care within the attached locality.

To understands population health needs within attached locality and develops collaborative working and maintains networks within the community, acting as a key stakeholder to ensure these needs are met.

To contribute to service/policy development through active membership on working parties or professional forums. To be aware of national and local policy that impacts upon the health and wellbeing of service users.

To review incidents and share any learning identified.

Qualifications

Essential criteria
  • Professional NMC Nursing Registration
  • Principles of physical assessment
  • V300 prescribing
Desirable criteria
  • Clinical Leadership
  • NHS Leadership academy qualification

General

Essential criteria
  • Master's degree or equivalent extensive experience in adult nursing
  • Current NMC registration
  • Full UK driving licence and vehicle
  • Proficiency in IT usage
Desirable criteria
  • Advanced skills in excel, word and powerpoint

Experience

  • Experience leading a team
  • Evidence of continued professional development
  • Experience nursing patients with complex needs in the community
