Job overview

We are delighted to offer a fantastic opportunity for a HCPC Registered Occupational Therapist to join our team for a short-term post to cover maternity leave cover, either as a fixed term contract or as a secondment. You will work as part of our Tamworth & Lichfield Discharge to Assess Team (D2A), a well-established and supportive integrated multi-disciplinary team of Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Social Care Practitioners, Home First Support workers and administrators. 

Ideally you will be a band 6 with D2A experience, but we will also consider applications from band 5 and band 6 OTs who have relevant skills and experience working in the community. 

You will be well supported by a Clinical Lead along with the D2A management and supportive Multi-Disciplinary Team who will offer continuous support and guidance to help achieve your goals.

Main duties of the job

As part of this role you will focus on providing comprehensive assessments and rehabilitation to support the individuals you see following their discharge home from hospital to maximise their potential, improve their independence and reduce the need for reliance on long term care and support.

You will be expected to:

  • Undertake a range of occupational therapy assessments to support maximising patients' independence after a stay in hospital.
  • Develop therapy goal plans for individuals which will then be shared with the designated care team to follow.
  • Review of the goal plans and adapting to meet the needs of the individual.
  • Liaise with other professional in the team around individual patients.
