Job overview

o

·           The role supports the Acute Care at Home in driving transformation as well as value for money in planning, commissioning and service delivery.

·                The role is designed to support the safe transfer of care from Acute Hospital wards, portals and departments into a Virtual Ward. Working closely with the Virtual Ward Matrons to deliver Case Management, Bed Management and Acuity Management of the patients suitable for Supported Early Discharge.

·                To work as part of a team reporting to the Senior Advanced Liaison Practitioner between the Acute Trust and Virtual Wards, in a “pull” function, to ensure the correct utilisation of Virtual Ward beds.

·                Identifying suitable patients who may need ongoing Virtual care within their own homes, reviewing clinical diagnostics and decision making to ensure the safe transfer of care. Ensuring Medical Management plans are appropriate and in place for discharge.

·                The job description and person specification are an outline of the tasks, responsibilities and outcomes required of the role.  The job holder will carry out any other duties as may reasonably be required by their line manager.

discharge pathways that includes engagement with Emergency Acute  Wards.

 

 

Main duties of the job

·               Operate effectively in a flexible and demanding environment and proactively engage with Acute Trust wards, portals, and departments.

·               Work effectively with a variety of external partners/stakeholders working on other projects and programmes within other NHS directorates as required.

·                Having the ability to deal with resulting potentially aggressive/antagonistic situations.

·               Committed to working and engaging constructively with internal and external stakeholders on a range of business sensitive issues.

·               Nurtures key relationships and maintain networks internally and externally, including national networks.

·               Close liaison with the Acute Hospital Site Team and Integrated Discharge Hub (IDH)

·               Link with managers and members of other functions, to address inter-dependencies and ensure alignment.
