Job overview
An exciting opportunity has arisen within our proactive Critical Care Outreach Team. The Critical Care Outreach team at the West Suffolk NHS trust is a passionate team of Nurses who have a varied background, and we are looking for staff to join the team.
We are seeking passionate and motivated staff who have experience in acute nursing, within an acute trust.
The successful candidate will need excellent communication skills and will need to prioritise their workload as will be carrying out assessment and delivering care to deteriorating patients trust wide leading to early intervention and treatment, autonomous working is therefore essential.
Formal and informal education is an integral part of the role and you will take an active part in education and training of the staff within the trust around the management of deteriorating patient, trust induction and any in house education.

Main duties of the job
Assessing patients who have been referred to the CCOT by trust wide MDT members.
Communicate with MDT to ensure care is delivered in a timely manner, leasing with Critical Care as required.
Active part of the cardiac arrest team, attending Major Haemorrhage calls and Traumas.
Supporting paediatric teams with any paediatric emergencies.
Active involved with education of trust staff.
Manging critical care outreach caseload.

- Assess acutely ill or deteriorating patients based on wards or appropriate departments and assist with their management plan and appropriate level of care, either avoiding admission to critical care or assisting with escalation to a higher level of care if necessary, in conjunction with the medical teams
- Ensure timely referral and admission to the appropriate critical care area for those patients who require a higher level of monitoring or clinical support than can be provided at general ward level
- Share knowledge and skills with clinical staff, both at the bedside and through formal education programmes within the Trust.
- With members of the multi-professional teams, respond to requests for support in assessing and managing the deteriorating and/or sicker ward patients.
- Manage caseload Outreach patients with the aim of avoiding admission to critical care, however facilitating timely admission where necessary.
- Following-up patients recently transferred out of Critical Care. To provide expert advice and education to clinical areas and all staff involved with patient care of this group of patients.

Education and qualifications
Essential criteria
- Registered Nurse
- Evidence of on-going professional development
- Evidence of working within an acute area
- Degree level study
Desirable criteria
- ALS Acute Care / Critical /cardiac Care Course
- Physical Assessment Course
Experience and knowledge
Essential criteria
- Evidence of continuing professional development
- Worked within an acute setting (I.e. ICU, CCU, ED, acute ward)
Desirable criteria
- Experience Providing Education and/or Training
- Previous experience or involvement in research / audit projects.
Skills and abilities
Essential criteria
- Excellent clinical skills
- Ability to care for a critically ill patient with no immediate supervision.
- Ability to care for critically ill patient in ward setting (outside of ICU)
- Ability to assess care needs, prioritise and allocate resources
- Effective interpersonal, organisational and team work skills
- Effective written communication skills
- Ability to learn and develop new skills and abilities
- Ability to cope under stress
- Ability to carry out formal and informal teaching to all levels of staff
- Demonstrates understanding of good practice in customer care
- Ability to motivate, support and encourage staff either clinically or within educational arenas

- Experience of improvement work and demonstrates involvement in QI initiatives

- Experience of change management
- Understanding of Comprehensive Critical Care/ NICE CG50 and other government directives, reports and current issues
