Job overview

We are seeking an experienced, dedicated and compassionate Band 6 Registered Children’s Nurse to join our Children’s Continuing Health Care Team as a Nurse Assessor.

In this specialist role, you will work within the framework of the NMC scope of professional Practice, delivering high-quality, family centred care assessments for children and young people with complex and continuing health care needs.

  • Work in close collaboration with families, health professionals, education providers and social care teams undertaking assessment and reassessment of children with complex and long term health needs, including those who are dependent on technology for survival.
  • In line with the National Framework  for Children and Young People’s Continuing Health Care (CHC).
  • Supporting the Complex Care Team Lead in the development and delivery of a range of services / packages of care for children and young people eligible for CHC.
  • Ensuring the effective coordination of care packages across health, education and social care, promoting a multi-agency, child-focused approach.
  • Your work will help ensure that children will receive the appropriate level of care and support to meet their individual needs.

Main duties of the job

·       Undertake Continuing Care assessments/reviews for children with complex needs in relation to care packages, respite care and placements for continuing care.

·       Collating and undertaking comprehensive and coordinated assessments within the framework of NHS Continuing Care for children and families within health and social care.

Attend the Children’s Complex Case panel  presenting

·       Hold own case load of continuing care clients ensuring timely reviews are conducted and ensure care packages continue to be effectively commissioned and are cost effective.

·       To communicate effectively with children/young people and their families from different cultures, in different contexts when assessing continuing care.

·         To demonstrate high level of specialist knowledge of children with complex needs and provide clinical advice to the multi-professional team and other professionals when required.

·         Undertake Continuing Care assessments/reviews for children with complex needs in relation to care packages, respite care and placements for continuing care.

·         Collating and undertaking comprehensive and coordinated assessments within the framework of NHS Continuing Care for children and families within health and social care.

·         Attend the Children’s Complex Case panel presenting cases and making recommendations regarding Continuing Health packages of care in line with the National Framework. and present robust and accurate clinical information.

Person specification

Education and qualifications

Essential criteria
  • RN – Child Branch
  • RSCN Diploma/Degree
  • Evidence of further study at degree/masters level
Desirable criteria
  • Specialist Practice Community Children Nursing Degree or equivalent

Experience and knowledge

Essential criteria
  • Minimum of 2 years post registration experience in acute or community setting
  • Working in partnership with the Multi-disciplinary team
  • Family-centered care
  • Excellent knowledge of safeguarding policies and procedures
Desirable criteria
  • Experience of service user engagement and patient satisfaction
  • Clinical audit and implementing change

Skills and abilities

Essential criteria
  • Excellent verbal and written communication skills.
  • Knowledge of NHS Continuing Care for Children and Young People’s Framework.
  • A sound clinical knowledge and competency base, in order to ensure scrutiny of compliance with statutory guidance and best practice
  • A background that demonstrates a wide spectrum of nursing experience, including inter-agency and multi-agency
  • Understanding of effective service development and ways in which to improve clinical care
Desirable criteria
  • Nursing Care of children and young people with complex health care needs
  • Acting in senior roles
  • Nursing care of children and young people in the community setting
  • Carrying out supervision and appraisal
  • Management experience including the day-to-day clinical management of a team within the acute or community setting

Personal qualities

Essential criteria
  • Commitment to work towards the Specialist Practice Community Children’s degree or other approved qualification
  • Excellent understanding of children/young people in need and those in need of protection
  • Good organisational skills

Other Requirements

Essential criteria
  • Frequent Travel Essential - You will need to travel, so you must either hold a full, current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
  • Ability to work flexibly including weekends, out of hours and bank holidays as necessary
