Job overview

An exciting opportunity for Community Nurses has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way.

The successful applicant will join a dynamic, friendly team of community nurses, community matrons, therapists and social care staff. You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. You will be assessing the patients by carrying out home visits and planning and implementing a patient’s individualised care plan. As part of the team you will also be delivering care to virtual ward patients.

On site at the health centre you have access to a range of treatment  facilities and may be involved in delivering nursing care in specialist clinics. You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience of palliative care, wound management and catheter management  will be an advantage.

Main duties of the job

·       To assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.

·       To work within the integrated team to facilitate early discharge from hospital.

·       To work within the integrated team to prevent unnecessary admission to hospital.

·       To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.

STAFF CAR PARKING

 

Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 – 18:30.

 

For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital.  Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.

 

This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete “on call” duties within their role.

  • RN Qualification (Part 1 of register) or equivalent.
  • Current NMC registration
Desirable criteria
  • Leg Ulcer Management
  • IV Administration /cannulation
  • ENB 998/mentorship preparation or equivalent
  • Mentorship qualification is essential for the post, but not essential at interview.

Experience & Knowledge

  • Able to communicate effectively using the English language (written and verbal)
