An exciting opportunity for a Physiotherapist who has experience of, or is interested in community based working, has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care so we are able to work together in a fully integrated way.
The successful applicant will join a dynamic, friendly team of therapists, community nurses and social care staff. You will be providing Physiotherapy to patients in the community and have the chance to work in an autonomous manner carrying your own caseload of patients with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. The role includes provision of therapy for patients with long term conditions but you will also be providing a responsive, same day service to patients to facilitate hospital discharges and carry out joint health and social care assessments of need in the community. On site at the health centre you have access to a range of treatment facilities and well equipped gym to provide individual and group therapy

You will be using a wide range of Physiotherapy skills and will need high level assessment skills. Experience of neurological conditions, musculoskeletal conditions, frailty, falls, dementia and orthopaedics will be an advantage. You will have opportunities to develop your role in accordance with areas of clinical interest, training and education are available to support this. We work closely with our Quality Improvement team and there are many opportunities for you to initiate projects which will lead to service improvement. Your additional duties will include the supervision and mentorship of junior staff and students and you will be involved in ongoing learning and development within the team.
This is a full time post. You will need to be a car owner and driver. We operate a 7 day service within core hours 8am to 6pm.
