Investing in our patient community, empowering them to live well‘ is the team vision which we work collectively to achieve.  The Sutton Community Neuro therapy & Adult Speech & Language therapy service is a Multi disciplinary team offering specialist, comprehensive assessment & intervention.  We work with adults in the Sutton area of South West London who have Neurological conditions &/or those with acquired communication, speech, eating, drinking and swallowing difficulties.   

Prif ddyletswyddau'r swydd

·       To provide specialist Speech and Language therapy assessment, differential diagnosis and treatment for a caseload of adult patients with acquired communication and/or swallowing disorders within community settings.

·       To demonstrate clinical effectiveness by use of evidence based practice and outcome measures in the areas of swallowing and communication disorders.

·       To hold and manage a specialist caseload and be legally accountable for all clinical decisions and professional actions, seeking advice from senior colleagues as appropriate

·       To identify SMART goals and  offer an appropriate therapy programme, taking into account the level of understanding, motivation,  insight, level of support and self-management potential.

Principal duties to include:

  1. Provide highly specialist SLT services to adults referred to the community service.
  2. Provide specialist and multi disciplinary assessment of adults taking into account all necessary factors including premorbid skills and environmental factors.
  3. Provide differential diagnosis of the communication/swallowing impairment, clearly define strengths and needs, identify SMART goals and offer an appropriate therapy programme, taking into account the level of understanding, motivation, insight, level of support and self-management potential.
  4. Develop and provide SLT training within the department and to other professionals as well as to families/carers and SLT students (to degree level), SLT assistants and support workers.

To demonstrate a well developed knowledge of paediatric therapy, underpinned by current evidence based practice.

· To manage a clinical caseload as an autonomous practitioner.

· To develop the ability to reflect on auditory, visual and kinaesthetic aspects of a client’s communication and to identify appropriate strategies to facilitate and enhance communicative effectiveness.

· To use knowledge to inform sound clinical judgements / decision making for case management.

· To make appropriate differential diagnoses based on assessment findings.
