Job overview

We are looking to recruit a Geriatrician with varying interests and skills to enhance and develop our current service provision. The department currently provides specialist services for falls, iOPAL, acute frailty, community services (Locality Hubs), dementia, Parkinson’s disease & Orthogeriatrics. The trust is keen to develop an outstanding geriatric service and is looking for a highly motivated and innovative physician to supplement the present team.  

The SAMS department has a history of innovative working both within the hospital and into the community:

•             Won the prestigious HSJ Award for Managing Long Term Conditions for our Nursing Home Project in 2012

•             Won the RCP Excellence in Patient Care Award in 2017 for our early supportive discharge strategy involving a multidisciplinary team in the hospital and in the community

•             Older Peoples Short Stay Unit a finalist in the HSJ Value in HealthCare awards in March 2018

•             2019 Finalist at the HSJ Value Awards and Patient Safety Awards

•             2018 PENNA Awards winner

We have multiple high-quality publications in renowned geriatric journals.

Main duties of the job

  • Work with existing consultants and the multi-disciplinary team to provide a high-quality, well-led service for patients of Ashford & St Peter’s.
  • Deliver Direct Clinical Care to patients, through all parts of the Department assisting clinical decision-making and sharing equally in the workload.
  • To have continuing responsibility for the care of patients in their charge and to undertake the administrative duties associated with their care across 7 days.
  • To be part of the GIM On call rota during their rotation in the Trust.
  • Have a continuing responsibility for the care of patients in his/her charge, ensuring safe handover as appropriate.
  • Engage with medical, nursing, management and all relevant co-workers in the delivery of department objectives: to provide clinical leadership, improve the patient experience; ensure the highest quality clinical care.
  • Actively participate in and support other staff with education, research, audit and clinical governance activities involving a multi-disciplinary workforce across the department
  • Participate in the professional training of the staff in the department, both medical and non-medical, and contribute as required to teaching sessions for other hospital staff as well as undergraduate and postgraduate staff, including supervision of junior medical staff.  Teaching and training will require verification of teaching certification

Detailed job description and main responsibilities

The Job Plan has a maximum of 9 PAs of Direct Clinical Care (DCC) including admin and 1 Supporting Professional Activities (SPA) which will be organised on an 18-week rota, and an additional 1.1 PAs for GIM work. The rota allows for consultants to alternate working on busy and less busy days in order to equalise workloads. 

Majority of the DCC is covering the ward in one of our SAMS wards  providing an in-reach service to surgical wards, or providing frailty cover to ED Department. 

This is an indicative weekly timetable for the current staffing of 10 consultants.  A specific job plan will be agreed upon with the appointed consultant to meet the needs of the service.  The job plan will be reviewed annually or upon request.

Essential criteria
  • Full GMC registration with a licence to practice
  • CESR registration at the time of starting the role

Experience

Essential criteria
  • Broad and comprehensive training in General Internal Medicine (GIM) and Geriatric Medicine.
  • Recent and relevant experience in managing elderly patients, including frailty, falls, dementia, polypharmacy, and end-of-life care.

Clinical skills & knowledge

Essential criteria
  • Ability to take full and independent responsibility for clinical care of patients.
  • Demonstrated knowledge of evidence-based practice and clinical governance.
