Job overview

Community Associate Practitioner  - 3 x Full Time, Band 4, Permanent. 

To work as part of the multi-disciplinary team, delivering health and social care, focusing on the recovery of people experiencing mental health problems.

Working autonomously with an allocated caseload, under supervision of a qualified clinician.

Main duties of the job

Assist with the running of therapeutic groups

Participate fully in the delivery of the Care Programme Approach as delegated by the Care Co-ordinator and pro-actively engage with individuals in their care.

Document and maintain accurate clinical records within the trust’s electronic records system. To support the qualified clinicians in completing key aspects of data entry on Systemone.

To work autonomously, with the ability to reflect upon clinical with the supervising clinican/s

  • Full and Valid UK Driving License with access to a vehicle for work purposes.

Qualifications

  • Foundation Degree in Mental Health or related subject
  • OR Level 3 Qualification Health related area and 2 years’ experience in a care role
  • OR 5 years’ experience in a role delivering care in a mental health setting

Experience

Essential criteria
  • 2 years’ experience working in a role delivering care in a mental health setting
