We have an exciting opportunity to recruit a new Mental Health Practitioner in Fanham PCN through Additional Roles Reimbursement Scheme (ARRS).

We are seeking to find Mental Health Practitioners who can come and make a difference. Providing high quality interventions in our new primary care integrated mental health service, you will be an important member of our primary care team in the Primary Care Network (PCN - which is a partnership of a few GP Practices).

The introduction of these roles is part of a wider transformation and expansion of community-based mental health services set out in the NHS Long Term Plan. The practitioner can access and draw down on secondary mental health services and help provide a bridge between primary care and secondary mental health providers.

The current Vacancy is a Full-Time (37.5 Hrs, 9am-5pm, Mon-Fri, working from the GP Practice/s), 7 Month Fixed term role at Band 6 in Farnham PCN.

The post-holder in this Vacancy will be required to work from 4 GP Surgeries within this PCN while a single GP Surgery address is given on the advert as an example.

Main duties of the job

• The practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team. Working alongside the other roles, they will also be part of the community mental health Multi-Disciplinary Team (MDT) with ready access to a wide range of support and expertise across both MDTs. It is envisaged that the focus of a practitioners work will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services. The practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway.
• Successful candidates must be experienced mental health practitioners and have experience of working in a Primary Care and/or Community Mental Health Team setting. You will be a confident communicator with colleagues and stakeholders alike. Above all you will have the ambition and drive needed to work within an environment of change and innovation.

As this role is based within a community setting we do require that you must be able to travel between multiple sites and trust locations within the set hours of your working day.
