Job overview

We're in the unique position of being able to advertise for a B7 Physio Team Lead to join our established MDT in ERS (Enhanced Recovery and Support at Home), part of the wider Intermediate Care Team.

Alongside the OT Team Lead and Nursing Team Lead, you will jointly lead the service both clinically and operationally. We pride ourselves on our culture of Continuous Improvement, championing new ways of working to support patients at home.

Patients are either at risk of admission to hospital or we have facilitated their early discharge home. We are a short term rehab service supporting patients to return to as independent level of functioning at home as possible. 

You will hold your own small, complex caseload and have line management responsibility for your team of Physios, who in turn line manage junior staff including Rehab Support Workers. Every Physio will have at least one student each academic year and we submit therapy requests to the Trust's CPD bid annually, including universal and therapy-specific masters modules. 

In September we have a member of the team commencing their Physio Apprenticeship and another commencing their OT Apprenticeship. This will be our second OT Apprenticeship.

There are other Physio Team Leads within Intermediate Care, one in each Community Therapies (long term rehab) and Community Inpatients, meaning you will have peer support and peer clinical supervision both immediately within the team and as part of the wider community services.

Main duties of the job

  • To provide clinical and professional leadership within Community Services including complex risk assessment;
  • To work in partnership with other Team Leads to provide comprehensive leadership, performance and clinical governance;
  • To hold a small, complex clinical caseload incorporating assessment, diagnosis, treatment and recovery plans.
  • Contribute to the development of service strategy and clinical standard operating procedures.
  • Engage with local networks in partnership with other health and social professionals/agencies and national links with other generalists in order to develop protocols according to national and local guidelines for the safe and effective provision of community therapies.

Detailed job description and main responsibilities

  • To hold a small, complex, clinical caseload that will include undertaking generic and specialist assessments of patients with diverse and complex physical conditions, providing a clinical diagnosis and delivery of individualised recovery programmes. The caseload will include patients requiring chronic disease management as well as support to avoid admission to hospital and on discharge from hospital due to acute medical change.
  • To support the recovery process by providing expert clinical advice and recovery to patients with multiple long term conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission or an unnecessary longer length of stay.
  • Provide high level assessment to establish most appropriate intervention, balancing clinical and managerial responsibilities to the service.
  • Ensure a high standard of clinical care for all patients and to support, facilitate and develop less experienced staff to do likewise.
  • Undertake comprehensive assessment of patients including those with diverse and highly complex presentations and multiple pathologies, using advanced clinical and analytical skills to provide an accurate assessment of their condition. 
  • Assess the physical and psychosocial needs of a defined client group.
  • Maintain a patient focus and ensure that patient experience is listened to and valued.
  • Discuss all recovery options with sensitivity, knowledge and expertise and to act as patient advocate when appropriate and respecting patient confidentiality with privacy and respect for diverse cultural backgrounds and requirements.
  • Provide individualised recovery plans for patients with highly complex multi-factorial long term conditions, so that their condition remains as stable as possible and, where appropriate, hospital admission is prevented and early discharge facilitated.
  • Analyse complex patient situations establishing a therapeutic relationship in which they are able to utilize counselling skills to assist the individual to adjust to their illness and care

Qualifications

Essential criteria
  • • Physiotherapy degree or diploma equivalent.
  • • Registered Physiotherapist with the HCPC
Desirable criteria
  • • NHS leadership training

Experience

Essential criteria
  • • Proven skills in leading, managing and developing teams
  • Relevant specialist/clinical experience and competence including: • Advanced clinical practice.
  • • Leading complex recovery/care co-ordination.
  • • Proactively manage complex long term conditions.

Knowledge

Essential criteria
  • • Highly specialist knowledge in managing patients in the community with complex recovery needs and long term/chronic disease management.
  • • Knowledge of current political drivers, health and social care issues.
  • • Highly developed knowledge of general management practice, including organisational performance management, finance, human resources and equality and diversity.
Desirable criteria
  • • Clinical experience of different care sectors, eg acute, Adult Services, community.

Skills

Essential criteria
  • • Highly specialist clinical and technical skills in recovery and rehabilitation for people with highly complex long term conditions and disabilities.
  • • Excellent written and verbal communication skills.
