Are you looking for a role where you can really make a difference to our Cancer Patients experience with Frimley Health?

This role will support the coordination of care for cancer patients through their cancer pathways from pre-diagnosis to end of treatment.  This role will be focussed around the telephone support of the cohort of patients who are referred to Frimley Health from local GPs, internal services and other trusts.

This role will be guided and supervised by the Macmillan Information and Support Officer in collaboration with the Head of Nursing Cancer and Macmillan Information & Support Manager. 

The post holder will coordinate the care and support given to cancer patients, their families and health care professionals who contact the Macmillan Cancer Navigator service at Frimley Health. The remote cancer navigation service will act as a telephone based single point of access for people affected by cancer from pre-diagnosis to end of treatment and support.  The post holder will also support recording of national audit data for cancer patients in collaboration with clinical teams to support various elements of the cancer pathway.

Main duties of the job

Co-ordinate care by providing a telephone/email based single point of access, for patients, carers and health care professionals from pre-diagnosis to treatment and living with and beyond cancer, and throughout their cancer journey.

Supporting Cancer Clinical nurse specialists by taking on administrative roles related to supporting Holistic Needs Assessment, documenting, tracking and recording, all patient and service users interactions. 

The post holder will also provide information to patients and service users regarding support available during their cancer pathway. 

Providing holistic support and information and signposting to existing support in the hospital and community. Working alongside registered practitioners to improve care for people with cancer, working closely with Clinical Nurse Specilialists and Allied Healthcare Professionals.
