Job overview

The Frailty Team at Frimley Park Hospital are seeking an organised, proactive, team player to join the Frailty Team as the administrator for 25 hours a week on a 12 month fixed term contract. This is a patient facing role to support in the daily administration tasks of the Same Day Emergency Care Department as well as data based in relation to data collection and analysis related to team performance.

The Frailty Liaison Team is commissioned to reflect the needs of the local community by providing a Same Day Emergency Care Service to assess the needs of patients presenting with frailty syndromes to the Emergency Department. Service aims include the assessment and coordination of care needs through a Comprehensive Geriatric Assessment and adoption of a co-produced approach with patients and service users.

The Integrated Frailty Liaison team, whilst operating from within the acute Trust, delivers a joined-up approach across the local Frimley Health and Care Integrated Care System (ICS), with the ambition to deliver high quality excellent care for people living with frailty.

Main duties of the job

  • To manage the administration of the Frailty Liaison Team and the services the team provide.
  • To support in the daily administration and flow of the Frailty Same Day Emergency Care (SDEC) Department, tasks include (but not limited to):
  • Becoming a super user in EPIC to support the team with any EPIC related queries.
  • Being a point of contact on the SDEC for any patients/visitors including answering phone and relaying any messages to the appropriate clinician.
  • Using EPIC to track patients onto the SDEC to support accurate patient location and data records.
  • To manage frailty team’s records and databases.

Detailed job description and main responsibilities

  • To provide administrative support for the Consultant Nurse for Older People, Advanced Clinical Practitioners, Frailty Practitioners, in-reach GPs and the Consultant Geriatrician team.
  • To manage frailty team’s records and databases including:
  • Developing, using and maintaining databases and spreadsheets containing patient details and outcomes.
  • Responsibility for ensuring accuracy of patient records, updating them with new or corrected information as required.
  • Sharing information/documentation with community partners as necessary, within data protection guidelines.
  • Producing and sending letters to GPs.
  • Follow-up phone calls to patients who have been discharged from hospital.
  • Use of EPIC to track patients, supporting the team with the admitting and discharging of patients from the SDEC to ensure accurate data.

  • GCSE level of qualification or equivalent

  • Experience of communicating with members of the public, face to face and on the telephone
Desirable criteria
  • Experience working in a large, diverse organisation such as the NHS or other public sector organisation

  • Computer higher level skills e.g EXCEL, Outlook, PowerPoint, Publisher and Word
  • Excellent written and verbal communication skills including face to face email and telephone
  • Problem solving skills
  • Ability to prioritise large volumes of work and work to deadlines with minimal supervision
