Job overview
We are delighted to be recruiting to our substantive Urgent Community Response (UCR) Therapy Team, where you will be one of at least three B7 Therapists, supported by non-registered members of the team.
With a passion for urgent patient care in the community, this is the perfect role for anyone excited by complex clinical situations, with varied and at times unpredictable workload.
Under the umbrella of Intermediate Care, UCR will be encouraged and facilitated to adjust and develop to the needs of the local population. You will receive clinical supervision within UCR, working in conjunction with Hospital at Home, and have regular 1:1s with the Service Manager. We will support your ongoing CPD through both formal relevant education (Masters) courses and informal opportunities across the ICS.
The service operates between 8am-8pm 7 days (8am-8pm), A current UK driving license is essential. Candidates with a driving license will require business insurance and will be asked to drive the Trust van and/or pool cars.
We will happily accommodate visits to the team, any time within current working hours.
Main duties of the job
- 2 hour response to patients at risk of admission to hospital
- First assessment of clinically unwell patient, in lieu of GP, Paramedic etc
- Specialist, complex therapy interventions for immediate safety and wellbeing
- Complex manual handling
- Integrated working with medical and emergency teams
- Signposting for ongoing treatment and preventative input

Detailed job description and main responsibilities
- Complete specialist, complex assessments demonstrating specialist knowledge of the concepts of biological, physical, social, and psychological sciences in relation to function to ensuring patient safety in their own homes
- Complete comprehensive holistic assessments to address immediate presenting needs of the patient utilising extended clinical skills
- Complex manual handling including urgent prescription of specialist equipment to manage immediate risks to patients during a crisis situation
- Profession-specific assessments, generalist assessments and, where appropriate and within scope of extended practice, cross-profession assessments to maintain patient safety during acute medical / social crisis
- Refer to appropriate clinical services to ensure patient safety is maintained; escalation of patient deterioration
- Use assessment tools/skills that will ensure an appropriate level of recovery intervention so that patients who present with highly complex needs are timely referred to the appropriate clinicians/teams
- Complete complex risk assessments, both as an independent clinician and drawing on the wider MDT as appropriate
- Triage and prioritisation of new referrals and allocation of workload
- To be responsible for the day-to-day management and prioritisation of your own caseload
- Respond to urgent referrals and an unpredictable workload: this will require prolonged concentration in an unpredictable environmen
- Be wholly accountable for your clinical practice taking every reasonable opportunity to sustain and improve knowledge and professional competence
- To manage a varied caseload, seeking support from peers and senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice
- To be actively involved in service development projects towards the development and delivery of UCR, therapy and broader community services, and to contribute to new developments and initiatives within the locality
- Play an active role in the management of clinical governance and patient safety

Qualifications
Essential criteria
- Degree or equivalent in Occupational Therapy or Physiotherapy
- Evidence of continued CPD with varied evidence
- Knowledge in identifying and prescribing appropriate equipment required in enabling patients whilst promoting safety
Desirable criteria
- Educated to masters level
- NHS leadership training
Experience
Essential criteria
- Extensive community experience at band 6 or band 7 level
- Experience working in admission avoidance
- Comprehensive MDT working
- Experience in carrying out clinical audit and risk assessment
Desirable criteria
- Experience of rapid response
- Managerial/supervisory experience and/or training
Knowledge
Essential criteria
- Knowledge of relevant national and local policies/guidelines relevant to this role
- Specialist knowledge of governance and risk strategies and their application in a community setting
- Highly specialist knowledge in managing patients in the community with complex recovery needs and acute and long term/chronic disease management
- Ability to adapt to the demands of a constantly changing environment
