Detailed job description and main responsibilities

To work as a clinical specialist practitioner, at advanced level, within the NMC Code of Conduct and Scope of Professional Practice
To manage the TVN team and work collaboratively with nursing and medical staff to ensure the planning and delivery of holistic care to patients with compromised tissue viability.
To develop partnership working with the multidisciplinary teams (Consultants, Physiotherapists, Occupational Therapists, Dieititian, etc) within the Trust.
To act as an expert practitioner in all aspects of Tissue Viability practice working with staff and patients, including sharp debridement
To be the Trust lead for local and national developments relating to tissue viability care, ensuring the Trust reviews its processes against NICE recommendations and guidance
To provide expert nursing input and leadership in ensuring a high standard of education and training in all areas of d tissue viability such as prevention dn management of pressure ulcer and wound care preceptorship study days.
To be the Trust lead for the CQUIN PU National Audit, ensuring trust participation and the development of an action plan once the results are known.
Review and update policies, procedures and standards in relation to tissue viability issues, ensuring policies are patient-centred, research based and effectively implemented 
To provide expert advice, at advanced level to patients, relatives and other health care professionals, on aspects of tissue viability care / management.
In conjunction with the ward sister, matron and Tissue Viability nurses. to carry out root cause analysis on patient who acquired new pressure ulcers and develop action plans for recommendations for future practice changes, ensuring that learning is shared throughout the trust.
Communicating with patients and staff in ways which empower them to make informed choices about their healthcare and wellbeing
To liaise with other experts on Tissue Viability (locally and / or nationally) to ensure development of best practice within the Trust and within the context of the whole health economy.
To be an expert link with Surrey Safe Care and to ensure the wound care assessments pressure ulcer risk assessment and care plans are accurate and evidence-based.  

To provide leadership, management support and supervision for the Band 6 nurses within the team.
To ensure structures are in place for the effective recruitment and retention of staff including family friendly and flexible local practices, which also take into account service, need.
To lead the recruitment process for the band 6 nurses as required
To manage staff absence including annual leave and study leave, sickness and maternity leave.

To implement, maintain and participate in  individualised performance reviews and objective setting for any direct reports, identify training and development needs including compliance to SaMT.
To implement and maintain the annual performance review of the nurses within the team, identifying any training needs and contributing to the Training Needs Analysis for the team
To ensure annual review of preparation and compliance to NMC nurse revalidation.
To ensure that the environment and staff comply with Health & Safety, Infection Control & COSHH regulations and that Trust Policies and procedures are followed correctly.
To ensure that incidents and accidents are reported and investigated according to Trust policy and that appropriate clinical risk management structures are in place
To encourage staff and maintain highest standards of personal and professional conduct and performance, participating in the disciplinary procedure according to Trust policy
To be aware of the broader Trust wide issues and resources, work as part of the managerial team in the nursing and patient experience division
To lead Quality Improvement agenda and support of team members to complete their QI projects.
To Lead on the completion of monthly reports on wounds/ pressure ulcers in the Trus.
To analyse the data reports, recognise indicators, identify actions and monitor implementation to ensure continuous improvement  and quality of care
Lead on all tissue viability related policies, pathways and guidelines. 
To work closely with the matrons, ward sisters/charge nurses to ensure appropriate support and supervision is provided in the clinical environment to ensure safe environments for our patients.

Develop, implement and participate in education and training programmes on tissue viability for all staff disciplines. Identify virtual teaching package as well as face to face
Identify and respond to any appropriate educational needs within the Trust, enabling practitioners to develop clinically based competencies
Develop and lead a team of tissue viability link nurses providing a structured development programme for this team
Develop and review competencies related to tissue viability.
To ensure training content incorporates learning from errors, incidents and patient feedback internal and external to the Trust. Ensuring patient stories are captured within the training
To act as a mentor to identified individuals in the clinical areas.
Evaluate effectiveness of the training and review training content as required.
Lead, manage, co-ordinate and present on the Trust training and clinical update training to all staff groups. Develop and evaluate educational programme to reflect best clinical practice.  
To provide best practice advice and support for clinical areas.
Represent the Trust by speaking at national conferences
To ensure patient and user involvement and feedback focus in relation to education and training.
Deliver training on 1:1 basis as well as formal training sessions. 
To ensure the training delivered meets the requirements of those working within both acuty and community settings
Contribute to clinical governance and clinical effectiveness agendas relating to tissue viability, implementing and evaluating best practice guidelines in tissue viability to improve patient care.
Examine and develop consistent research and evidence based practice to support clinical effectiveness within the speciality.

To lead on any policy development within key areas of knowledge 
To lead on the review of nursing documentation that links withTissue Viability e.g. PU risk assessment tools and individualised care plan.
To ensure all training content and delivery is research based/ best practice, planned and evaluated.
Actively seek to improve practice through application of research and audit activities.
To identify and intervene where circumstances contribute to an unsafe environment for patients and staff; escalating this to the appropriate members of staff.
To be fully aware of government reports, nursing and patient experience strategies, and corporate/business unit objectives, in relation to this role.  In conjunction with the multi-disciplinary team use available resources to meet these targets/implement change.
To identify actions needed as a result of the results from the national pressure ulcer audits.
To critically evaluate research evidence and apply these to clinical practice when appropriate.
To lead on research and audits which aim to collect evidence to support and strengthen clinical practice 
To ensure patient and user involvement and feedback in evaluating the service
