Job overview

Royal Surrey Hospital NHS Foundation Trust is looking for a proactive and organised administrator to support our newly established and innovative Transfer of Care Hub (ToCH) and Integrated Neighbourhood Team (INT) Hubs. Working alongside clinical leads, social care services, and wider community teams, this role will play a vital part in ensuring patients have a smooth and efficient discharge from hospital, and ensure clear coordination of community based services that will support this.

This post is only open to staff from Royal Surrey NHS Foundation Trust and its system partners (Ashford and St Peters NHS Foundation Trust, Surrey Heartlands ICB, and Surrey County Council). Applicants from outside of these organisations will not be considered.

Main duties of the job

The postholder will provide high-quality administrative support across the Transfer of Care Hub and Integrated Neighbourhood Team (INT) Hubs to ensure smooth co-ordination of hospital discharges and effective functioning of community-based, multidisciplinary services. Working closely with clinical leads, social care teams, voluntary sector partners, and operational managers, the postholder will support the development of integrated working practices in community services, robust discharge planning, and timely communication to improve patient outcomes and system flow. 

The postholder will work closely together where one will have a primary focus on ToCH admin support and the other INT admin support. The postholders will be required to cover for each other in times of leave.

Detailed job description and main responsibilities

Key responsibilities of the role:
•    Provide high-quality administrative support to the ToCH and INT Hubs.
•    Coordinate daily discharge planning meetings and follow-up actions.
•    Lead on maintaining accurate data on discharge tools, trackers, and caseloads in the form of Microsoft Excel spreadsheets and patient tracking lists.
•    Support MDT meetings, prepare agendas, take minutes, and manage action logs.
•    Liaise with health, social care, and voluntary sector partners to enable integrated working.
•    Act as a first point of contact for referrals and service queries.
•    Ensure compliance with information governance standards.

Qualifications

Essential criteria
  • • GCSEs (or equivalent) including English and Maths
Desirable criteria
  • • NVQ Level 3/4 or equivalent in Business/Admin

Knowledge

Essential criteria
  • • Understanding of the NHS and adult community services; Awareness of Information Governance and Data Protection
Desirable criteria
  • • Knowledge of the Surrey Heartlands integrated care system and how partners interact at place level
  • • Awareness of hospital discharge policies and integrated care models
