Job overview

An exciting opportunity for a band 5 community nurse has arisen within our East Surrey Community team for adults within Surrey. As a team we continually strive to develop, and we are seeking an enthusiastic and highly motivated nurse to join our dynamic multi- disciplinary service.

The successful applicant will be joining an innovative team of nurses providing high quality health interventions as part of the wider forward thinking, diverse multi-disciplinary team, working collaboratively with nurses and other professional clinicians as well as multiple external agencies.

 

Main duties of the job

As an individual we will expect you to possess excellent communication skills, enjoy working as part of a team, working closely with colleagues from a range of disciplines, whilst also being able to work independently. You will be expected to carry out comprehensive assessments; plan, implement and evaluate delivery of nursing interventions as part of a holistic approach. You will also be able to demonstrate that you are able to apply your nursing skills to a wide range of situations, working with a range of adults with a learning disability living within the community. You will be responsible for managing a caseload across the geographical area of East Surrey, with full supervision and support from senior nurses and be expected to work closely with other agencies as well as members of the multi-disciplinary team. All applicants will either be registered on Part 5 or 14 of the NMC Register or be completing training that will result in registration.

A key responsibility is promoting health and independence for people with learning disabilities by delivering high quality nursing assessments, interventions and consultation under supervision within a Positive Behaviour Support framework within generic and specialised services; and providing supervision and guidance to support workers.  This will be undertaken within an accessible, non-stigmatising, localised service that contributes to the specialist health care of people with learning disabilities within the locality.

  • RNLD Nursing Degree/Diploma

Experience

  • 1 year experience of working with people with learning disabilities (can be pre-qualification)
  • Experience of Multi Disciplinary working

  • Experience of working in a learning disability health setting (placements can be included)
Desirable criteria
