Job overview
A unique and exciting opportunity has arisen for a highly motivated and enthusiastic individual to join our dynamic multi-disciplinary community team as a Community Support Worker. The post holder will have experience working with people with a learning disability, be able to manage and prioritise a caseload whilst supporting implementation of care plans to a high standard as developed by experienced clinicians including Nurses, Occupational Therapist, Dietitians and Speech and Language Therapists. The post holder will be required to work autonomously whilst demonstrating good interpersonal skills, including verbal and written.
This post requires elements of practical work – including but not limited to : teaching to maximise someone's independence, assistance with practical health issues – supporting health appointment, provision of advice, development of accessible information. The post holder may be required to support physiotherapy which involves water-based therapy and, the successful candidate will be supported to access relevant life saver training. The post holder will be supported to access robust supervision and relevant training opportunities.
The team covers the East of Surrey, with bases at both Redhill and Leatherhead. Due to the Community aspect of the role and travel between bases, the successful applicant must have a full clean UK driving license and a vehicle that can be used for work purposes.
Main duties of the job
· To provide practical support and follow a plan of care developed by a team professional.
· To participate in assessments, planning, implementation and evaluation of interventions as part of a holistic approach working with the team of clinical professionals in a variety of settings.
· To feedback to the professional on the implementation and evaluation of the work that they are undertaking.
· To observe and document any changes in the health and wellbeing of the people that they are supporting, reporting to the clinical professional/professional team any new or change in risk or presentation of the person.

Essential criteria
- Valid UK driving license and access to a vehicle for work purposes
